射频治疗肝癌术后复发的相关因素 被引量:5

摘要 射频消融治疗肝癌已经成为目前治疗肝癌的主要非手术方法之一,但术后较高的复发率受到关注。本文根据近年来的文献,对射频毁损术后复发的危险因素做一综述。结果显示,肿瘤直径>3cm、肿瘤位置临近大血管或肝脏包膜下、不足够的治疗范围、以及术者经验缺乏是使局部复发率升高的危险因素,而血清甲胎蛋白水平高、术前肝功能分级差、肿瘤病理分期晚等因素是肝内远处复发的危险因素。 Liver cancer is one of the most popular malignancies of human beings. We are threatened by its high morbidity rate and mortality rate. Although surgery has been the chief way to treat it, few patients have the chance for rad- ical resection. Radiofrequeney ablation has become one of the main non-surgical techniques for treating liver cancer, but the recurrence rate of patients who have undergone radiofl'equeney ablation is relatively high. Local recurrence and intrahepatic distant recurrence are the most common types of recurrence. We reviewed the correlation factors for recurrence of liver cancer after radiofrequency ablation. Tumor diameter more than 3cm, proximity to large vessels, subeapsular location, lack of margin and physicians' experience are risk factors for local recurrence. High levels of AFP, poor Chile-Pugh grade and increased TNM grade are risk factors for intrahepatic distant recurrence.
