干细胞移植治疗缺血性心脏病的安全性 被引量:1

摘要 干细胞移植途径有经冠状动脉(冠脉)内、经静脉、采用电机械标记系统(NOGA)经心内膜下注射、心外膜下心肌内注射及自体移植等多种方式,且安全、可行,可以改善缺血性心脏病患者的心功能。其中,心外膜下心肌内注射最为直接可靠,便于术后细胞分化鉴定,但由于需要开胸手术,对某些心力衰竭(心衰)晚期患者可能难以接受,且有诱导心律失常、引起细胞回漏等副作用。经心内膜下肌内注射方法具有不需要开胸手术的优越性,但却要求有相当高的心血管介入手术技术,注射位置较难固定,有导致心律失常和心脏穿孔的潜在危险,因高压注射器可引起移植细胞坏死。经冠脉内注射移植方式较为直接,同时创伤较小,是目前最常用的移植方式,但是这种移植方式需要较高浓度的细胞,而且必须在介入操作情况下才能完成。经静脉途径接近于无创,但是大量细胞可能随着血流迁移至其他器官,引起远隔器官不必要的血管新生,可能导致血管瘤、视网膜血管增生等并发症。刘克强等采用自体骨髓干细胞动员联合生长激素的方法治疗大鼠急性心肌梗死(AMI),毛细血管密度明显增加。多数缺血性心脏病患者在进行骨髓干细胞移植治疗的同时也进行了冠脉介入治疗,
