摘要
目的探讨良性前列腺增生患者经尿道前列腺气化电切术后尿管牵引止血的有效牵引重量。方法186例经尿道前列腺气化电切术后患者随机分为3组各62例,各用200 g、350 g、450 g重量牵引尿管,观测尿液中红细胞计数、发生尿管堵塞的情况,评价牵引的效果。结果术后48 h,用200 g重量牵引的患者,其冲洗液中红细胞计数最多,平均每高倍视野52个,发生尿管堵塞9例;用350 g和450 g重量牵引的患者,平均每高倍镜视野红细胞分别是25个和23个,尿管堵塞发生率均为0。在冲洗液中红细胞计数方面,200 g牵引组较其他两组明显增加,差异有统计学意义(P<0.0001),350 g牵引组和450 g牵引组间差异无统计学意义(P>0.05)。结论经尿道前列腺气化电切术后患者牵引尿管止血的效果不是随牵引重量增加而提高,其最佳牵引重量为350 g。
Objective To explore the effective traction weight for the hemastasis by urinary canal traction for patients undergoing transurethral electrovapor-resection of the benign prostate hyperplasia. Methods One hundred and eighty-six patients undergoing transurethral electrovapor-resection of prostate were divided into three groups at random, 62 in each. The urinary canals of the three groups were tracted with the weight of 200 g, 350 g and 450 g, respectively. Then erythrocyte count in the urine was determined and blocking in the urinary canal was checked to assess the traction effects. Results The 48 h after the operation, the erythrocyte count in the flushing fluid of the 200 g-traction-weight patients was the greatest, averaging 52 per high power field, and 9 cases of urinary canal blocking occurred in this group. The erythrocyte count for the 350 g- and 450 g-traction-weight group were 25 and 23 per high power field, respectively, and no incidence of urinary blocking was observed. The erythrocyte count in the flushing fluid of the 200 g- traction-weight group was distinctly higher than that of the other two groups, having statistical significance (P〈0.0001), while the differences between 350 g- and 450 g-traction-weight were not statistically significant. Conclusion The hemastasis effect of urinary canal traction for the patients undergoing transurethral electrovapor-resection of the prostate is not proportional with the traction weight, with the best being 350 g.
