单剂量卡托普利对高血压患者血小板内游离钙的影响 被引量:2

摘要 目的研究血小板内游离钙离子浓度变化和高血压进程的关系以及口服单剂量卡托普利对高血压患者血小板内游离钙的影响。方法测定102例不同分期的原发性高血压患者静息血小板内游离钙浓度,其中52例Ⅰ~Ⅱ期的轻中度高血压病人口服卡托普利25mg,测定给药前和给药1h后血压(BP)、血浆肾素活性(PRA)、血管紧张素Ⅱ(AngⅡ)和血小板内游离钙离子浓度(Ca2+)。结果原发性高血压患者静息血小板内Ca2+明显高于正常血压对照组。静息血小板内Ca2+和高血压分期有明显的相关性。静息血小板内Ca2+随高血压分期的增高而升高;口服单剂量卡托普利1h后BP、血浆AngⅡ水平和血小板内Ca2+明显降低,而血浆PRA则明显升高。血浆AngⅡ和血小板内Ca2+下降值之间(r=0.37,P<0.01)及二者与平均压下降值之间均有弱的相关性(r=0.38,r=0.36,P<0.01)。血浆AngⅡ和静息血小板内Ca2+之间无相关性。结论原发性高血压患者血小板处于激活状态,并和高血压进程相关。口服单剂量卡托普利可导致血小板内Ca2+下降。 Aim\ To investigate the effects of single dose of captopril on intracellular free calcium concentration([Ca 2+ ]i) of platelets and relationship between the activation of platelets and the progression of hypertension in hypertensive patients.\ Methods\ The resting [Ca 2+ ]i of platelets was measured in 102 essential hypertensives(EHT),of which 52 cases were orally given a single dose of captopril.\ Plasma renin activity(PRA),angiotensin Ⅱ(AngⅡ) concentration and [Ca 2+ ]i of platelets were monitored before and 1 hour after administration.\ Results\ It was found that the basal [Ca 2+ ]i level of platelets was significantly higher in the patients with EHT than in normal controls,and was steadly increased with the progression of hypertension.\ The blood pressure,plasma AngⅡ,and resting Ca 2+ of platelets were markedly decreased 1 hour after administration of a single dose of oral captopril,while PRA was significantly increased.\ There were weak correlations between the decreases in mean BP and the decreases in the Ca 2+ of platelets(r=0 36,P<0 01),and the decreases in plasma AngⅡ(r=0 38).\ A weak correlation between the changes in plasma AngⅡ and the decrease in Ca 2+ was also noted(r=0 37,P<0 01).\ No significant correlation was found between plasma AngⅡ and the resting level of Ca 2+ of platelets.\ Conclusion\ Platelets are activated in the patients with hypertension and the activation of plateletes are paralelled with the progression of hypertension.\ Plasma AngⅡ seems responsible neither for the maintainance of basal level nor for the changes of Ca 2+ of platelets after administration of oral captopril in the hypertensives.
