92例B系急性淋巴细胞白血病多参数流式细胞术免疫表型分析 被引量:2

Immunophenotyping of 92 Patients with Acute B Lymphoblastic Leukemia by Multiparameter Flow Cytometry

摘要 目的探讨B系急性淋巴细胞白血病(B-ALL)患者的免疫表型特点及其临床意义。方法采用高分辨多参数流式细胞术CD45/SSC散点图设门方法,对92例初发性B-ALL患者的骨髓幼稚细胞表面及胞内分化抗原进行分析。结果B-ALL患者的免疫表型具有明显特征:HLA-DR、CD10、CD19、CD22、CD34、CD38、cCD79a、TdT最为常见;髓系抗原CD11b、CD13、CD15和CD33亦可见明显表达,表达率分别为16.7%、42.4%、17.4%和37.4%。不同亚型比较,前前B-ALL中HLA-DR、CD10、CD19、CD22、CD34、CD38、cCD79a的表达率均在90%以上,而不成熟B-ALL不表达CD10,CD11b、CD13、CD33主要在前前B-ALL上表达,CD15则在前B-ALL中多见。儿童B-ALL的免疫表型与成人B-ALL之间没有显著性差异。结论B-ALL的异常骨髓细胞具有明显的本系列特征、原始细胞标记和跨系表达。且不同免疫亚型的B-ALL具有一定的表型差异。 Objective To evaluate the specificity and clinical significance of acute B lymphoblastic leukemia (B-ALL) immunophenotyping. Methods Multiparameter flow cytometry and CD45/SSC gating were used to analyze the surface and cytoplasmic antigen expression of the bone marrow blasts in 92 cases of primary B-ALL. Results In 92 B-ALL patients, HLA- DR, CD10, CD19, CD22, CD34, CD38, cCD79a and TdT were the most common markers; Of the myeloid antigens, CD11b, CD13, CD15 and CD33 expressed more obviously. The expression rate of HLA-DR, CD10, CD19, CD22, CD34, CD38 and cCD79a in the pre-pre-B-ALL was over 90%. No CD10 was detectable in immature-B-ALL. CD11b, CD13 and CD33 were expressed mostly in pre-pre-B-ALL, and CD15 was mainly in pre-B-ALL. The immunophenotypes presented no remarkable difference between children and adults. Conclusion The abnormal bone marrow cells in B-ALL showed B-lineal features, blast markers and trans-lineal expressions. And different subtypes had various immunotypes.
