远端带筋膜皮下组织瓣的腓肠神经营养血管皮瓣临床应用 被引量:12

摘要 目的总结远端带筋膜皮下组织瓣的腓肠神经营养血管皮瓣修复合并空腔形成的皮肤软组织缺损的疗效及特点。方法2004年12月-2007年5月,行远端带筋膜皮下组织瓣的腓肠神经营养血管皮瓣修复10例皮肤软组织缺损。男8例,女2例;年龄5~70岁,中位年龄29.5岁。皮肤软组织缺损位于后足跟、跟底7例,外踝及跟外侧2例,小腿下1/3段及踝后区1例。缺损范围8cm×6cm~11cm×11cm。伤后9~42d进行手术。术中皮瓣切取范围10.0cm×8.0cm~13.0cm×12.5cm,所带筋膜皮下组织瓣范围7.0cm×1.5cm~8.0cm×5.0cm。供区行全厚皮片植皮修复。结果术后8例皮瓣完全成活,创面Ⅰ期愈合,无感染;远端1cm表浅坏死及内侧部分坏死各1例,对症处理后愈合。供区均Ⅰ期愈合。术后10例获随访1~13个月。皮瓣外观较满意,感染无复发,无慢性溃疡及压疮形成。结论远端带筋膜皮下组织瓣的腓肠神经营养血管皮瓣具有容易控制创面感染,保留足跟负重面功能的优点,适于修复有空腔形成的足跟部创面。
