摘要
目的了解责任护士对层级全责护理模式的认同情况,进一步推进层级全责护理的深入发展。方法采用自行设计问卷对某三级甲等医院196名责任护士进行层级全责护理模式的认同度调查。结果91.9%的责任护士认为提高了护理质量,89.8%认为提高了工作积极性,98.0%认为对病情更熟悉,90.3%认为心理护理、健康教育较前落实,88.8%认为基础生活护理较前落实,89.3%认为护理记录书写较前更有连续性,92.3%认为护患关系较前改善,86.7%认为医护配合程度提高;而55.1%认为自我成就感无明显提高,94.9%认为在层级全责护理模式下工作压力增大。结论实施层级全责护理在提高护理质量方面得到了责任护士的认同,但护士的自我成就感低、工作压力增大。建议在职教育应高度重视对护理人员职业价值观和整体素质的培养;医院建立合理的激励机制;合理配置人力资源;提供减压知识和技能培训教育。
Objective To investigate the identification of primary nurses on full responsibility gradation nursing mode so as to promote its development. Methods A self-designed questionnaire was used to investigate the identification degree on full responsibility gradation nursing mode among 196 nurses from a Class III Grade I hospital. Results 91.9% of the prima~ nurses thought the mode worked to have increased the nursing quality, 89.8% though it enhanced their work enthusiasm, 98.0% thought it facilitated them with patient's conditions, 90.3% thought psychological care and health education were better implemented than before, 88.8% thought basic living nursing were better implemented than before, 89.3% thought the nursing records were written more continuously than before, 92.3% thought nurse-patient relation was better improved than before, 86.7% thought nurse-doctor cooperation was enhanced than before. However, 55.1% of them thought there were few improvements in auto-sense of achievement and 94.9% thought the mode raised work pressure. Conclusion The primary nurses have identification on full responsibility gradation nursing mode in respect of the improvement of nursing quality, but riley have a lower level for self sense of achievement and feel increased pressure of work. It is suggested to attach great importance to cultivate their concepts on occupational values and their holistic quality. Hospitals should regulate reasonable incentive mechanism, optimize allocation of human resources and provide them with knowledge of alleviating pressure.
