雷米芬太尼复合丙泊酚在胸腔镜肺大泡切除术中的应用 被引量:4

摘要 目的观察雷米芬太尼复合泊酚全凭静脉麻醉在胸腔镜肺大泡切除手术的应用效果。方法30例患者,男26例,女4例;年龄18~58岁,ASAI~II级,以雷米芬太尼1μg/kg,咪唑安定0.5~1.0mg/kg,丙泊酚1~2mg/kg,维库溴铵1.0~1.5mg/kg,诱导后气管插管,麻醉维持:雷米芬太尼0.5μg·kg-1.min-1,丙泊酚4~8mg·kg-1.h-1,微量输注泵恒速输入,间断追加维库溴铵。分别记录不同时点MAP、HR、SpO2、PETCO2变化。结果诱导期注入雷米芬太尼后MAP、HR明显下降(p<0.05),但诱导后各时点对比无明显差异,单肺通气期MAP、HR维持稳定,SpO2、PETCO2变化差异无统计学意义,术毕全组患者均在停止输注药物5~10min清醒,术毕拔管时间(6.20±4.8),全组无术中知晓发生。结论雷米芬太尼复合丙泊酚全凭静脉麻醉应用于胸腔镜下肺大泡切除术,镇静镇痛效果确切,可维持血流动力学平稳,起效快、作用时间短,但应加强术后早期镇痛。
