摘要
为评价吻合器痔上黏膜环切钉合术（PPH）联合改良外剥内扎术（Milligan-Morgan术）治疗重度脱垂性混合痔的临床效果，将227例Ⅲ，Ⅳ度脱垂性混合痔患者分为两组，治疗组121例采用PPH联合改良Milligan-Morgan术，对照组106例采用PPH。结果显示，手术后两组患者原有症状均得到了有效控制。治疗组较对照组的手术时间和创面愈合时间延长，但住院时间、并发症发生率无差异，术后疼痛指数、肛缘外残留皮赘发生率及患者满意率明显优于对照组。结果表明，PPH联合改良Milligan-Morgan术具有术后疼痛轻、肛缘外残留皮赘发生率及复发率低等优点，是治疗重度脱垂性混合痔的理想方法。
Two hundred and twenty seven patients with prolapsed and Ⅲ,Ⅳ stage mixed hemorrhoids were divided into two groups. 121 cases in the study group underwent PPH plus modified Milligan-Morgan operation,while 106 cases underwent PPH only（the controlled group）. As results,the original symptoms of the patients in both groups were all brought under control;while the therapeutic result of the aspect of operation duration of time and the wound healing time in the study group were inferior against those in the con- trolled group, that of the aspects of hospital stay and complication rate in the two groups were similar, and that of the aspects of postoperative pain index, the occurrence rate of perianal residual skin tag as well as patients＇ satisfaction rate in the study group were superior against that in the controlled group. It is concluded that PPH plus modified Milligan-Morgan operation has more strong points and is the optimal procedure of choice.
