混合核酸检测法筛查HBV窗口期献血者 被引量:7

摘要 窗口期、免疫静默感染以及罕见的亚型与变异是导致常规血清学漏检的主要原因。其中窗口期造成的影响尤为常见,有研究表明,在美国90%的HBV、HIV以及75%的HCV输血感染是因为窗口期漏检所致。核酸检测(nucleic acidtest,NAT)由于检测对象是核酸,而不是抗原或抗体,并且方法学与血清学检测常用的ELISA不同,具有窗口期短、灵敏度高的特点。一些发达国家从1999年开始在血液筛查中用核酸检测法。由于欧美国家HBV感染率很低,因此使用NAT主要是检测HCV、HIV。然而中国是乙型肝炎高发区,HBV感染率高达10%以上,漏检风险要远远大于HCV、HIV。青岛市中心血站使用核酸检测试剂对12000名无偿献血者常规采血进行HBV、HCV、HIV检测。过程中发现1例典型的HBV窗口期感染,现报告如下。
