血管病变3D DCE SCTA和3D DCE MRA与DSA对照研究 被引量:2

A Comparative Study on the Diagnostic Accuracy to Pathological Changes in Vessele Under 3D DCT SCTA,3D DCE MRA and DSA

摘要 目的:对照分析3D DCE SCTA(Ⅰ)、3D DCE MRA(Ⅱ)和DSA(Ⅲ)三种影像学检查在全身血管病变诊断中优势和不足,指导临床医师选择适合不同患者的最佳检查方法。材料与方法:对46例经手术证实的血管病变的患者进行Ⅰ、Ⅱ两种检查,传输到GE AW4.1工作站,利用VR、SSD、MIP、MPR等后处理技术,获得血管病变的3D图像,并将检查结果与Ⅲ的检查结果相比较。结果:46例患者中:Ⅰ检查的准确率95.6%,Ⅱ检查的准确率91.3%,Ⅲ检查的准确率97.8%。其中:脑血管病变:Ⅰ检查的准确率95.9%,Ⅱ检查的准确率为92.3%,Ⅲ检查的准确率97.5%。②肺动脉栓塞及下肢静脉血栓:Ⅰ、Ⅱ两种检查的诊断敏感性和特异度可达100%。③主动脉夹层:Ⅰ和Ⅱ的诊断符合率达100%。④上腔静脉阻塞:Ⅰ和Ⅱ与Ⅲ的符合率为100%。⑤肝癌门静脉癌栓:Ⅰ和Ⅱ对门静脉癌栓的诊断特异性89.2%,敏感性94.3%。⑥肾动脉瘤:Ⅰ诊断的敏感性94.1%;Ⅱ诊断的敏感性为100%。结论:3D DCESCTA、3D DCE MRA与DSA检查之间是相辅相成的,三者之间相互补充、相互促进,能够提高血管病变的诊断率及患者的生存率。 Purpose: Make a comparative analysis on the advantages and shortage of 3D DCE SCTA (Ⅰ), 3D DCE MRA ( Ⅱ ) and DSA (Ⅲ) three kinds of imageology check in wholebody having blood vessel disease, guide the best check method that the therapist chooses the feat and different suffere. Materials and Mefliods: To 46 patients of having blood vessel diseases confirm by sttrgical operation carries on two kinds of checks of Ⅰ , Ⅱ , transmitting the work station of GE AW 4.1, using back disposal technique of the VR, SSD MIP, MPR etc. acquire the 3D images of the pathological changes in vessels, and compare tbe check result and Ⅲ check result mutually. Results: In 46 cases, the accttrate rate of Ⅰ is 95.6%, Ⅱ is 91.3%, Ⅲ is 97.8%. Among them. ①Cerebral vascular diseases: The accurate rate of Ⅰ is 95.9% ; the accurate rate of Ⅱ is 92.3% ; and the accurate rate of Ⅲ is 97.5%. ②Pulmonary embolism and venous thrombosis: Two kinds of diagnosis sensitivity for checks of Ⅰ , Ⅱ and especially the difference can amount to 100%. ③Aortic dissection: The I matches with the Ⅱ diagnosis the rate to amount to 100%. ④Superior vena'eava tbrombus: Diagnosis accordance rate of Ⅰ and Ⅱ with Ⅲ is 100%. ⑤Portal cancerous thrombosis: The specificity of Ⅰ and Ⅱ to the diagnosis to hepatoma protal vein blot is 89.2%, sensibility is 94.3%. ⑥ Renal arterial aneurism: The sensilility that Ⅰ and Ⅱ examine a patient is 94.1% and 100%, respectively. Conclusion: 3D DCE SCTA ( Ⅰ ), 3D DE MRA ( Ⅱ ) and of the check of DSAs ( Ⅲ ) are complement each other, three and mutually and promote mutually, can raise the diagnosis rate of the pathological changes in vessels and the existence rate of the sufferers.
