急性心肌梗死患者窦性心率震荡变化的临床意义 被引量:11

摘要 目的观察急性心肌梗死患者窦性心率震荡的变化规律。方法选择83例急性心肌梗死患者(其中及时行再灌注治疗成功者37例),根据患者是否出现恶性室性心律失常急性左侧心力衰竭、心源性休克、心脏性死亡等将患者分为高危组30例和低危组53例;另选对照组57例。所有入选患者均接受动态心电图检查,测量窦性心率震荡的参数TO、TS及心率变异的参数SDNN等。结果急性心肌梗死患者窦性心率震荡明显减弱,其中及时行再灌注治疗组较未及时行再灌注治疗组明显改善。结论窦性心率震荡可作为判断急性心肌梗死患者成功再灌注治疗的参考指标,且有利于急性心肌梗死患者高危患者的检出,对其预后评估有较好的预测价值。 Objective To investigate changes of heart rate turbulence (HRT) in patients with acute myocardial infarction(AMI). Methods The parameters of HRT (turbulence onset and turbulence slope) and heart rate variability (SDNN) were measured from 24-hour ambulatory electrocardiogram in 83 patients with AMI (37patients underwent successful reperfusion therapy) and compared to that in 57 normal control subjects. Results HRT decreased obviously in patients with AMI and improved more significantly in patients with than without successful myocardial reperfusion. Conlusion HRT may be a useful indicator of successful reperfusion and help distinguish high risk patients after AMI.
