摘要
目的提高对颅底肿瘤治疗的认识。方法回顾分析显微外科手术治疗颅底肿瘤56例的临床特点和总结手术体会。结果56例颅底肿瘤中,手术全切除46例,次全切除4例,大部切除6例。术前颅神经功能损伤35例46条,术后恢复23条(50%),好转8例(17.4%),症状无改善9条(19.6%),加重6条(13.0%),出现新的颅神经症状者5例。结论术前精确的诊断对手术及预后非常重要。手术入路主要取决于肿瘤的位置、延伸及大小。显微外科手术是减少术后并发症、获得良好预后的关键。
Objective To enhance the knowledge on microsurgical treatment of base skull tumors. Methods The clinical characteristics and operating experiences of microsurgery treatment of base skull tumors were retrospectively analyzed. Results The total, sub total, and partial resection were carried out in 46, 4 and 6 cases respectively. Among the 3.5 cases, 46 pieces of cranial nerves were injured before operation, the function of injured cranial nerves recovered in 23 pieces （.50%）, improved in 8 pieces （]7.4%）, hardly improved in 9 pieces （19.6%）, worsened in 6 pieces （13.0%） after operation, and the new injury of cranial nerves occurred in .5 cases. Conclusions Precise preoperative diagnosis is very important for surgery and prognosis. The operative approach is mainly determined by location, extension and size of the tumors. Microsurgery is the key to reducing the postoperative complications and to obtaining the good prognosis.
