缺糖基转铁蛋白检测在酒精性肝病诊断中的应用价值 被引量:4

Applied Value on Determination of the Plasma Carbohydrate-deficient Transferrin Level in Cases of Alcoholic Liver Disease

摘要 目的检测缺糖基转铁蛋白(CDT)在酒精性肝病(ALD)患者血浆中的水平,并评价其在诊断ALD中的作用。方法采用乳胶颗粒增强的免疫散射比浊法、双波长速率法、血常规分析分别检测ALD患者、非ALD的其他肝病(NALD)患者及正常对照组血浆中CDT、AST、ALT、GGT水平和红细胞平均体积(MCV)。结果NALD组GGT、ALT、AST浓度明显高于正常对照组,差异有统计学意义(P<0.05)。ALD组%CDT、GGT、ALT浓度与正常对照组比较,差异有统计学意义(P<0.05);ALD组MCV、血浆AST浓度与正常对照组比较,差异无统计学意义(P>0.05)。ALD组与NALD组比较,%CDT显著增加,GGT、ALT、AST浓度较低,差异均有统计学意义(均P<0.05);MCV的差异无统计学意义(P>0.05)。结论%CDT可作为诊断酒精性肝病较为理想的检测指标。 Objective To determine the level of plasma carbohydrate deficient transferrin(CDT) and evaluate its application prospect in diagnosis of alcoholic lever disease (ALD). Methods The plasma CDT level was determined by turbidimetric immunoassay; the ALT,AST and GGT level were measured by double wave length speed assay and the MCV was gained from the EX-2000 blood cell analyzer. Results The level of plasma ALT, AST and GGT in cases of NALD were all higher than that in the healthy people (P〈0.05).The level of plasma CDT,GGT and ALT in these cases of ALD was significant higher than that in the healthy people too, meanwhile, there was no significant difference in MCV and the plasma level of AST between the cases of ALD and the healthy people.To compare with NALD, the plasma level of CDT increased evidently, but the plasma level of GGT,ALT and AST was decreased obviously in cases of ALD. There was no difference in MCV between the two groups. Conclusion The plasma level of CDT might be a potential and ideal index in the diagnosis of ALD.
