摘要
目的研究中西医结合"三活疗法"(TST)对股骨头缺血性坏死的临床疗效,探索控制坏死进展、缩短疗程、保全股骨头与恢复髋关节功能的方法。方法选择确诊的股骨头缺血性坏死(ANFH)164例为临床研究对象,追踪观察2年。TST组64例:‘一活’:经皮骨内减压生物载体成形术。‘二活’:同基因骨髓移植术。‘三活’:内服中药"骨脉通"与局部透入"痹特灵"。CP组48例:经皮骨内减压与‘三活’。CM组52例:即‘三活’。应用能够反映骨关节病变及临床表现的"四项五级"标准进行观察,四项作为临床定性观察指标,五级作为临床定量观察指标。疗效标准依Harris-赵德伟氏百分制评估。结果TST组总有效率89.1%;CP组总有效率85.4%;CM组总有效率82.7%。TST组分别与CP、CM组间比较总有效率差异无显著性(P>0.05),但TST组优良率高达82.8%,明显高于CP组的58.3%和CM组的51.9%(P<0.01)。达优良病例的时间:TST组为(260土53.9)d,CP组为(460士73.1)d,CM组为(490士81.2)d。组间比较差异有显著性(P<0.01)。TST组ⅢA期34例中有8例恢复到Ⅱ期;而CP组ⅡC期11例中有5例进展到Ⅲ期。CM组ⅡB-C期23例中有9例进展到Ⅲ期。结论初步体会中西医结合"三活疗法"集中西互补之优势,手术微创痛苦小,不良反应,疗效及方法独特,为骨坏死的治疗提供了新的思路和方法。
Objective To study integrated Chinese and western medicine ＂three live therapies＂ （TST） on the clinical effect of the avascular necrosis of the femoral head. Methods Select diagnosed avascular necrosis of the femoral head（ANFH） 164 cases for clinical research,TST group 64 cases：‘ one live＇ ： percutaneous bone decompression biocarrier performed. ‘two live＇ ： with bone marrow transplantation. ‘Three live＇ ： internal medicine,＂ Gu - Mai Tong＂ and the partial penetration of the ＂Bi - Te Ling＂. CP Group 48 cases ： percutaneous decompression and bone ‘ three live＇. CM Group 52 cases ：the ‘ three live＇. Applly the reflecting bone and joint diseases performance of the ＂four - five＂ standard, four as a clinical observation of qualitative indicators, five as a clinical observation of quantitative indicators. Clinical standards in accordance with Harris - percentile Diaodewei＇s assessment system. Results TST result, the total efficiency 89. 1% ;CP group,the total efficiency 85.4% ;CM group, the total efficiency 82. 7% ,there was no significant difference in efficiency among TST group,CP Group,CM group （ P 〉0.05） ,but the excellent rate of TST group 82. 8% ,significantly higher than that of the CP group 58. 3% and CM group 51.9% （ P 〈0. 01 ）. Conclusion Integrated Chinese and western medicine ＂three -live treatment＂ has advantages of minimally invasive surgical pain, less side effects, special efficacy.
