摘要
目的探讨幽门螺杆菌(Hp)感染对过敏性紫癜(HSP)患儿肾损害的影响。方法采用13C尿素呼气试验(13C-UBT)方法将2002年1月-2005年12月广州市儿童医院收治的HSP患儿304例(其中男176例,女128例;年龄3-14岁)分为Hp感染(A组)91例,无Hp感染(B组)213例,采用χ^2验比较二组间肾损害发生率;对HSP伴Hp感染患儿,进行驱Hp治疗,予2周的三联疗法:奥美拉唑(7岁及以下10mg/d,7岁以上15mg/d)分2次口服。克拉霉素片15mg/(kg·d),分2次口服。A组中有肾损害60例,经治疗Hp转阴45例(C组),未转阴15例(D组),采用χ^2验,比较C、D二组间肾损害恢复情况。结果A组肾损害的发生率为65.9%(60/91例),B组为35.2%(75/213例),二组比较有显著性差异(χ^2=24.378P〈0.01)。C组肾损害完全恢复38例将恢复率为84.4%,D组肾损害完全恢复7例,恢复率为46.7%。二者比较有显著性差异(χ^2=8.563P〈0.01)。结论Hp感染对HSP患儿肾损害的发生有明显的影响,及时发现和处理Hp感染有助于患儿肾损害的恢复。
Objective To evaluate the relationship between Henoch - Schonlein purpura (HSP) accompanying renal impairment and helicobater pylori (Hp) infection. Methods This study consisted of 304 patients with HSP. The patients were divided into 2 groups( group A and group B) based on Hp infection or not(91 cases in group A and 213 cases in group B). Compared with the rates of accompanying renal impairment in 2 groups. And observed the recovery from renal impairment between the patients who were turned into negative ( group C ) and patients still were positive after the anti - Hp therapy( group D). Numeration data were analyzed by χ^2 test. Results Group A which was with Hp infected,the accompanying renal impairment ratio was 65.9%. Group B which was without Hp infected,the ratio was 35.2%. There was significant difference between 2 groups ( χ^2 = 24. 378 P 〈 0.01 ). In group A ,60 patients who were HSP accompanying renal impairment with Hp infection, after anti -Hp therapy ,45 patients were negative in Hp infection( group C), and 84.4% recover from renal impairment, but 15 were still positive ( group D) and only 46.7% recover. The ratio of group C was higher than the group D ( χ^2 = 8. 563 P 〈 0.01 ). Conclusions Our study shows a significant correlation between Hp infection and renal impairment in HSP patients. And the treatment of anti - Hp in time may be helpful for the recovery from renal impairment in the HSP.
