摘要
目的:检测X线平片评价胸腰椎压缩性骨折的疗效,筛选X线平片评价指标,提出X线平片评价体系。方法:对60例(79椎)单纯胸、腰椎压缩骨折患者治疗前及治疗后2、4、12周X线平片的伤椎椎体前、后缘高度,Cobb角以及相邻椎体高度等指标进行测量,统计椎体高度的损失率、恢复率、矫正率、治疗后损失率及Cobb角矫正率,并将上述影像学观测指标与临床疗效进行比较。结果:在0、2、4、12周4个检测时相中,以第4周最有价值且具有代表性。该时相中60例压缩椎体前缘高度损失率、恢复率、矫正率治疗后椎体前缘高度损失率分别为(15.3±16.6)%、(97.4±3.5)%、(90.3±3.4)%、(4.2±1.9)%;Cobb角大小为(4.7±3.4)°;Cobb角矫正率为93.3%。结论:用X线平片评价胸腰椎压缩性骨折的疗效时,伤椎椎体前缘高度的改变最具有临床价值,Cobb角与椎体高度的测量价值相同;检测时相以0、2、4、12周为宜;伤椎高度损失率反映椎体的损伤程度,伤椎高度恢复率反映治疗效果,伤椎高度矫正率反映疗法的效率,伤椎治疗后损失率反映疗效的巩固程度;上述4个"率"构成胸腰椎压缩性骨折的X线平片疗效评价体系。
Objective:To detect curative effect of X-ray plain film on evaluating compression fracture of thoracolumbar centra,select index of it' s evaluation,and propose system of it's evaluation.Methods:60 cases of patients with simple compression fracture of thoracolumbar centra(79 pieces of body of vertebra) were chosed for observation,such indexes as pro-and posterior border height ofinjuried body of vertebra,cobb's angle' degree and height of close together body of vertebra on X-ray plain film were measured on times of pretreatment and two weeks,four weeks and twelve weeks post-treatment.Rates of losing,recovery,correcting,and losing post-treatment,rectification of cobb's angle were statisticed,indexes above were compared with those of curative effect.Result:On four points,that of four weeks was best.In all patients the items above were respectively(15.3 ±16.6)%,(97.4±3.5)%,(90.3±3.4)%,(4.2±1.9)%,(4.7±3.4)°,93.3%.Conclusion:In the method above,there had best clinical value on changes of anterior border height in injuried body of vertebra,it was the same for degree of cobb's angle on X-ray plain film and clinical measurement.They should be done on points of zero,two weeks,four weeks and twelve weeks.Lose rates of height on X-ray plain film reflect degree of injury of vertebral body,recovery rate of height lost was refer to therapeutic efficacy,correct rate of height in injuried body of vertebra was be to therapeutic efficiency,lose rate of injuried body of vertebra post-treatment was solidify degree of curative effect.So the rates above were the main parts of evaluation system on X-ray plain film on compression fracture of thoracolumbar centra.
