摘要
目的应用海德堡视网膜血流仪(HRF)研究中心性浆液性脉络膜视网膜病变(中浆)视盘筛板和黄斑部视网膜血流动力学变化。方法以22例(22只眼)中浆患者为研究对象(中浆组),运用HRF、眼底荧光血管造影(FFA)和眼底吲哚青绿血管造影(ICGA)检测视盘筛板和黄斑部视网膜的血流动力学指标。分别以患者另一只健康眼(中浆对照组,n=22)和正常眼(正常对照组,n=28)作为对照。结果HRF检测结果显示,与正常对照组比较,中浆组和中浆对照组的视盘筛板和黄斑部视网膜的血流量、流速及红细胞移动速率显著减低(P<0.05或P<0.01);而中浆组与中浆对照组各项检测指标比较差异无统计学意义(P>0.05)。中浆组和中浆对照组的FFA和ICGA表现与HRF检测结果相吻合。视盘筛板与黄斑部视网膜血流的相关性分析表明,两者的血流动力学指标呈显著正相关(P<0.01)。结论中浆患者的视盘筛板和黄斑部视网膜血流下降显著,且两者血流动力学改变存在正相关性。HRF用于评价脉络膜微循环状态有效且安全。
Objective To investigate the hemodynamics of sieve plate and retina in macula in central serous chorioretinopathy(CSC) with Heidelberg retinal flowmeter (HRF). Methods Twenty-two eyes of 22 patients with CSC were selected (CSC group), and HRF, fundus fluorescein angiography (FFA) and indocyanine green angiography (ICGA) were employed to detect the hemodynamics of sieve plate and retina in macula. The other 22 normal eyes of these patients were served as CSC control group, and another 28 normal eyes as normal control group. Results It was revealed by HRF that compared with normal control group, the blood flow volume, blood flow velocity and velocity of erythrocytes of sieve plate and retina in macula were significantly decreased in CSC group and CSC control group (P 〈0.05 or P 〈0.01). However, there was no significant difference in the parameters between CSC control group and CSC group (P 〉 0.05). The same findings of CSC group and CSC control group were obtained by FFA and ICGA. The correlation analysis indicated that the hemodynamic parameters of sieve plate were positively related to those of retina in macula ( P 〈 0.01 ). Conclusion The blood flow of sieve plate and retina in macula is significantly decreased in CSC, and there is positive correlation between hemodynamic parameters of sieve plate and retina in macula. HRF is safe and effective in the detection of microcirculation of choroid.
