血浆脑利钠肽水平与高血压病患者血压昼夜变化及左室肥厚关系的研究 被引量:1

摘要 目的:通过观察原发性商血压(EH)患者血浆中脑利钠肽(BNP)水平的变化,探讨其与EH患者24小时血压昼夜变化及左室肥厚(LVH)的关系。方法:对EH患者93例,对照组20例行24h动态血压监测,采用放射免疫分析法测定血浆BNP含量;同时测定其左窒质量指数(LVMI)。结果:(1)EH患者血浆BNP水平明显高于对照组(P<0.01);(2)与杓型组相比,非杓型组BNP水平升高更为明显(P<0.05);(3)两组EH中伴左室肥厚的患者血浆BNP水平无明显差异(P>0.05);(4)杓型EH患者血浆BNP水平与24小时平均脉压(24hPP)成正相关(r=0.56,P<0.01),非杓型EH患者则无明显相关性(P>0.05)。结论血浆BNP水平与EH患者血压昼夜变化异常和左室肥厚有关。 Objective :To investigate the relationship of B -Type Natriuretic Peptide level and the diurnal rhythm of blood pressure and left ventricular hypertrophy for essential hypertension patients. Methods: Ambulatory BP monitoring( ABPM )for 24 hours was performed in ninety - three patients with essential hypertension and twenty control subjects. Plasma BNP level in subjects was measured using radioimmunoassay method, left ventricular hypertrophy were measured too. Results: Plasma BNP level was higher in patients with hypertension than that in control group ( P 〈 0. 01 ). Compared with dippers, nondippers had a higher BNP plasma level (P 〈 0. 05). Among the two types of essential hypertension, patients with left ventricular hypertrophy had the same BNP level( P 〉 0. 05). There was significant correlation between logarithmic- transformed plasma BNP level( log BNP)and 24 hours mean pulse pressure in dippers ( r = 0. 56, P 〈 0. 01 ) , but not in nondippers ( P 〉 0. 05 ). Conclusion : Plasma BNP level was clinically useful for the early identification of hypertensive patients who had abnormal circadian BP variability and left ventricular hypertrophy.
