摘要
目的:探讨两种术后疼痛治疗管理模式对术后疼痛治疗效果及并发症发生率的影响。方法:2004年1-12月间行术后镇痛治疗的615例妇科手术患者为A组,术后疼痛治疗管理方法为一人一周法;2003年1-12月行术后镇痛的545例妇科手术患者为B组,术后疼痛治疗管理方法为一人一天法。比较两种管理方法下患者术后镇痛的VAS评分、镇痛效果评价、满意度评价以及并发症的发生率、提前中止镇痛发生率等情况。结果:A、B两组2d平均VAS评分没有明显差异(P>0.05);A组镇痛效果总体评估为"差"的病例数比率明显低于B组(P<0.05);A组患者满意度为"好"的病例数比率明显高于于B组,满意度为"中"、"差"的病例数比率明显低于于B组(P<0.05);两组患者镇痛并发症的发生率差异没有显著性(P>0.05),A组提前中止镇痛治疗的发生率明显低于B组(P<0.05)。结论:一人一周法比一人一天法在术后镇痛的管理上更完善,能提高患者镇痛满意度,减少提前中止治疗的发生率。
Objective To evaluate the effects of two modes of acute pain services management on the efficacy of post-operative analgesia and complications, Methods Group A includes 615 patients that received acute pain services during 1, Jan, 2004 to 31, Dec, 2004, the inspecting doctor shifted weekly, Group B includes 545 patients that received acute pain services during 1, Jan, 2003 to 31, Dec, 2003, the inspecting doctor shifted daily. The patients' VAS scores, the evaluation of pain controlling efficacy and their satisfaction, and the frequency of complications between the two groups were compared, Results There was no significant difference of VAS scores between group A and B in 2 days (P 〉 0.05), The number of patients that the pain controlling efficacy evaluated as ‘bad' was significantly lower in group A than that in group B (P 〈 0.05). In group A, the ratio of patients' satisfaction scored ‘good' was significantly higher than that in group B and the ratio of scored ‘moderate' and ‘bad' was significantly lower than that in group B (P 〈 0.05). There was no significant difference of the frequency of complications in both groups. The cases of patient discontinued analgesia ahead of schedule was lower in group A than that in group B (P 〈 0.05). Conclusion For post-operative analgesia management, the mode of weekly shift is better than the mode of daily shift, which can improve the satisfaction of patients and reduce the frequency of patients discontinue analgesia ahead of schedule.
