摘要
目的:比较临床上几种常用的颈部旋转手法对颈椎髓核内压力的影响。方法:用生物材料实验机对新鲜尸体颈椎标本进行定量旋转和牵引。实验状态分为:A状态:先以不同重量的牵引,再旋转颈椎;B状态:先旋转颈椎,再以不同的重量牵引;C状态:牵引和旋转同时进行。结果:髓核内压力随着牵引力的增加而下降,旋转时有轻度升高;在复位过程中压力出现一过性地下降。在A状态以200N的力牵引时,髓核内压力下降最明显;B状态下旋转时髓核内压力有轻度升高,牵引后有轻度下降;C状态基本对髓核内压力无影响。结论:在A状态下行颈椎旋转手法最安全。
Objective: to study on the effect of traction combined with rotatory manipulation on the pressure of cadaveric cervical nucleus pulposus. Methods: The cervical spine was tracted and rotated by material test system(MTS), in different states and pressure of cervical nucleus pulposus of C 3~4 ,C 4~5 and C 5~6 were measured respectively with pressure sensor . State A: under different tracting forces, and then rotated the cervical spine; State B: at first rotated the cervical spine, then tracted it; State C: tracted the cervical spine and rotated the cervical spine at the same time. Results:①Along with the tracted force increased, the pressure of cervical nucleus pulposus fell continuously, and the pressure increased slightly when rotated the specimen.②The felling remained a certain period of time when the state of cervical spine was restored.③In state A, the pressure fell down obviously and increased slightly under 200N tracting force, and the specimen was then rotated; In state B, the pressure increased in a certain extent firstly, and then fell down slightly; In state C, the pressure basically had no change. Conclusion: The rotatory manipulation under traction is the safest rotatory manipulation of cervical spine.
