2型糖尿病患者空腹血糖、糖化血清蛋白、血脂与糖尿病视网膜病变的关系 被引量:9

Relationship between fasting blood glucose,glycosylated serum protein,blood lipid and diabetic retinopathy in type 2 diabetics

摘要 目的探讨2型糖尿病患者空腹血糖、糖化血清蛋白、血脂与糖尿病视网膜病变(diabetic retinopathy,DR)的关系。方法将128例2型糖尿病患者按有无视网膜病变分为DR组66例、无DR组62例,与正常对照组58例进行比较,观察其空腹血糖(fasting blood glucose,FBG)、糖化血清蛋白(glycosylated serum protein,GSP)和血脂(TG、TC、LDL-C、VLDL-C、HDL-C)的变化。结果无DR组和DR组的FBG水平分别为(8.92±2.86)mmol·L-1和(9.24±3.11)mmol·L-1,二者相似(P>0.05),但均明显高于对照组(4.52±0.84)mmol·L-1(P均<0.01);无DR组和对照组的GSP水平分别为(250.2±43.6)μmol·L-1和(237.3±36.8)μmol·L-1,二者相似(P>0.05),但均明显低于DR组的(385.6±61.4)μmol·L-1(P均<0.01)。无DR组和DR组的TG水平分别为(1.96±0.86)mmol·L-1和(2.32±0.98)mmol·L-1、TC水平分别为(5.62±1.02)mmol·L-1和(6.36±1.08)mmol·L-1、LDL-C水平分别为(3.89±0.94)mmol·L-1和(4.21±1.02)mmol·L-1、VLDL-C水平分别为(0.54±0.04)mmol·L-1和(0.79±0.05)mmol·L-1,均明显高于对照组[TG:(1.36±0.67)mmol·L-1,TC:(4.13±0.96)mmol·L-1,LDL-C:(3.36±0.86)mmol·L-1,VLDL-C:(0.35±0.02)mmol·L-1;P均<0.01];无DR组和DR组的HDL-C水平分别为(1.02±0.03)mmol·L-1和(0.86±0.02)mmol·L-1,均明显低于对照组的(1.38±0.05)mmol·L-1(P<0.01);DR组TG、TC、VLDL-C均明显高于无DR组(P均<0.01),而DR组HDL-C明显低于无DR组(P<0.01),但2组LDL-C水平比较差异无统计学意义(P>0.05)。结论2型糖尿病患者FBG、GSP升高和脂质代谢紊乱与DR的发生密切相关。 Objective To investigate the relationship between fasting blood glucose ( FBG), glycosylated serum protein(GSP) ,blood lipids and diabetic retinopathy(DR) in non-insulin dependent diabetes mellitus(type 2 diabetics ). Methods According to a retinopathy examination, the 128 cases of type 2 diabetics were divided into the DR group (66 cases) and the non-DR group ( 62 cases) ,respectively. They were separately compared with the normal controls (58 subjects) and observed the changes in FBG, GSP and blood lipids. Results FBG levels in both DR group and non-DR group were ( 8.92 ±2.86 ) mmol· L^- 1 and ( 9.24 ± 3. 11 ) mmol· L^- 1, respectively. Although these two measurement did not illustrate a significantly difference ( P 〉 0. 05 ), they were significantly higher than in the normal controls, where FBG level was (4. 52 ± 0.84) mmol· L^- 1 (P 〈 0. 01 ). GSP concentrations both in non-DR group ( 250.2 ± 43.6 ) μmol · L^-1 and in the normal controls ( 237.3 ± 36.8 ) μmol· L^- 1 were similar ( P 〉 0.05 ), but they were significantly lower than in DR group ( 385.6 ± 61.4 ) μmol· L^- 1 ( P 〈 0.01 ). In non-DR group and DR group,where the level of TG were ( 1.96 ± 0. 86 ) mmol· L^- 1 and (2.32 ±0.98) mmol· L^- 1 compared with ( 1.36 ± 0.67 )mmol· L^- 1 in controls ;The level of TC were ( 5.62 ± 1.02 )mmol· L^- 1 and (6.36 ± 1.08) mmol· L^- 1 compared with (4. 13 ± 0. 96 )mmol· L^- 1 , in controls; The level of LDL-C were (3.89±0. 94)mmol· L^- 1 and (4.21 ± 1.02)mmol· L^- 1 compared with ( 3.36 ± 0.86 )mmol· L^- 1 , in controls ; The level of VLDL-C were(0.54 ±0.04) mmol· L^- 1 and ( 0.79 ± 0.05 )mmol· L^- 1 , compared with ( 0.35 ± 0.02 ) mmol· L^- 1 in controls. However,a reversed direction was observed in HDL-C with ( 1.02±0.03 )mmol· L^- 1 in non-DR group and ( 0.86 ± 0.02 ) mmol· L^- 1 in DR group compared with ( 1.38 ± 0.05 )mmol· L^- 1 in the controls(P 〈 0.01 ). A further laboratory test verified that the TG, TC and VLDL-C levels were significantly higher in DR group than in non-DR group(P 〈 0.01 ). However ,HDL-C concentration was significantly lower in DR than in non-DR(P 〈 0.01 ), and LDL-C concentrations in both DR group and non-DR group did not illustrate a significantly difference(P 〉 0.05 ). Conclusion A relationship between high FBG, high GSP and the metabolic disorder of blood lipids was confirmed in our clinic observational study.
