摘要
背景：全瓷修复体具有良好的生物相容性与耐腐蚀性，较之金属烤瓷修复体具有更好的美学效果等优点。目的：评价IPS—EmpressⅡ全瓷联冠固定夹板修复治疗前牙区牙周病时的生物相容性反应。设计、时间及地点：病例观察，于2005—02／2006—12桂林市口腔医院完成。对象：选择门诊就诊患者15例，男6例，女9例，年龄为21～67岁。方法：为15例成人慢性牙周炎患者制作了18件IPS—EmpressⅡ全瓷固定夹板。主要观察指标：分别于修复后即刻，3，6，12个月采用美国加利福尼亚牙科协会评价标准评价修复体效果。各观察时期基牙的菌斑控制指数、探诊后出血、牙周袋探诊深度、临床附着水平、固定夹板的松动度及牙槽骨支持指数。结果：随访12个月，15例患者均进入结果分析。通过统计学分析，固定夹板修复治疗后不同观察时间的临床检查指标之间差异无显著性意义：基牙的牙周袋探诊深度的变化无显著性改变。固定义齿功能状态良好，无一例固定夹板出现松动现象。全部18件全瓷联冠式固定夹板均无修复体边缘变色、表面无明显磨损，患者牙龈无炎症水肿等过敏反应发生。修复体的满意率为93.3％。结论：IPS-EmpressⅡ全瓷联冠固定夹板与宿主未发生特殊生物相容性不良反应，近期修复效果良好。
BACKGROUND： All-ceramic prosthesis possesses good biocompatibility and corrosion resistance, and is superior to metal ceramic prosthesis regarding the appearance.
OBJECTIVE： To evaluate the biocompatibility of IPS-Empress Ⅱ all-ceramic crown fixed prosthesis in patients whose anterior teeth suffered from chronic periodontitis.
DESIGN, TIME AND SETTING： A case observation was carried out in the Guilin Stomatological Hospital from February 2005 to December 2006.
PARTICIPANTS： Fifteen outpatients were admitted in Guilin Stomatological Hospital, including 6 males and 9 females, who aged 21-67 years.
METHODS： Fifteen adult patients of chronic periodontitis were restored with 18 IPS-Empress Ⅱ all-ceramic fixed splints.
MAIN OUTCOME MEASURES： Immediately, and 3, 6, 12 months after the restoration, the periodontal conditions were evaluated by plaque control record, bleeding on probing, periodontal probing depth, clinical attachment loss, mobility of fixed partial denture and bone support. The performance of the patients was evaluated according to California Dental Association criteria.
RESULTS： Fifteen patients were all followed up for 12 months. There were no significant differences in the clinical indexes and the periodontal probing depth of the abutment after the fixed splint restoration. Fixed prosthesis exerted good functions, and no case appeared splint loosening. No discoloration or abrasion was noted in 18 all-ceramic fixed splint prostheses. No case appeared gingival inflammation. The satisfactory rate of the fixed splints was 93.3%.
CONCLUSION： The short-term clinical behavior of IPS-Empress Ⅱ all-ceramic fixed splint is encouraging, without any adverse effect of the biocompatibility.
