摘要
目的探讨肝外胆管癌组织中环氧化酶-2(cyclooxygenase-2,COX-2)与基质金属蛋白酶-9(matrix metalloproteinase-9,MMP-9)的表达与肝外胆管癌临床病理特征之间的关系及其预后意义。方法采用免疫组织化学法检测69例肝外胆管癌,12例非肿瘤胆管黏膜上皮组织中COX-2与MMP-9的表达情况,并分析其与临床病理参数以及预后的关系。结果69例胆管癌组织,12例非肿瘤胆管黏膜上皮组织中COX-2表达阳性率分别为79.7%(55/69)、33%(4/12);69例胆管癌组织,12例非肿瘤胆管黏膜上皮组织中MMP-9表达阳性率分别为85.5%(59/69)、41.7%(5/12),有统计学意义。COX-2、MMP-9的表达与TNM分期有密切关系。单因素分析表明MMP-9阳性表达与肝外胆管癌预后负相关。结论COX-2、MMP-9表达与肝外胆管癌的临床病理特征密切相关,有助于预后的判断。
Objective The aim of this study was to evaluate the expression of cyclooxygenase-2 ( COX-2 ) and matrix metalloproteinase-9 ( MMP-9 ) in extrahepatic cholangiocarcinoma(EHCC), reveal its relation to clinicopathological factors and investigate its impact on prognosis. Methods The expressions of COX-2 and MMP-9 were detected by immunohistochemistry in 69 cases of EHCC and 12 cases of non-tumor bile duct epithelium. The relationship between COX-2 and MMP-9 expression,clinicopathological parameters and prognosis were observed. Results The expression rate of COX-2 in 69 cases of EHCC, 12 cases of non-tumor bile duct epithelium was 79.7~ and 33~ respectively( P d0.05). The expression rate of MMP-9 in 69 cases of EHCC, 12 cases of non-tumor bile duct epithelium was 85.5% and 41.7% respectively. There were significant difference between EHCC and non-tumor bile duct epithelium in COX-2 and MMP-9. COX-2 and MMP-9 expression was associatied with TNM clinical stages respectively. According to univariate survival analysis, MMP-9 expression correlated with poor prognosis( P 〈0.05). Conclusion The expressions of COX-2 and MMP-9 were significantly correlated with the clinicopathological characteristics and biological behaviors of EHCC. Detection of COX-2 and MMP-9 is valuable for evaluating prognosis of EHCC.
