摘要
目的探讨BNP与Swan-Ganz漂浮导管所测得的各项血流动力学指标的相关性。方法选取2007年1~10月入住我院心内科的23例住院患者做血清BNP定量检测并置入Swan-Ganz漂浮导管,测定中心静脉压(CVP),肺动脉压(PAP),肺毛细血管压(PCWP),计算心脏指数(CI),每搏搏血量(SV),左室每搏做功指数(LVSWI),收集数据做多元回归分析,用ANOVA方差分析做相性分析。结果23例患者BNP均数=(266.19±362.51)pg/ml;CVP均数=(5.46±5.01)mmHg,与BPN的相关系数R=0.30,P=0.03;PAP均数=(21.26±9.81)mmHg,与BNP的相关系数R=0.09,P=0.16;PCWP均数=(13.86±10.65)mmHg,与BNP的相关系数R=0.16,P=0.03,CI均数=(2.26±0.56)L/(min·m^2),与BNP的相关系数R=0.08,P=0.20;SV均数=(51.79±13.70)ml/beat,与BNP的相关系数R=-0.08,P=0.79;LVSWI均值=(30.62±14.16)g/(m·m^2),与BNP的相关性R=-0.01,P=0.93。结论BNP的水平与有创血流动力学指标检测相比有简便、无创、迅速经济的优势,可以大大提高临床做出诊断的效率。
Objective To explore the correlation between BNP level and parameters of invasive hemodynamic examination with Swan-Ganz's flowing balloon catheter.Methods 23 patients were measured plasma brain natriuretic peptide(BNP)levels and measured invasive hemodynamic parameters with Swan-Ganz catheter,such parameters include CVP、PAP、PCWP、CI、SV and LVSWI.All data were analysed by multiple regression and ANOVA test.Results The average level of BNP is(266.19±362.51)pg/ml;mean CVP=(5.46±5.01)mmHg,R=0.30,P=0.03;mean PAP=(21.26±9.81)mmHg,R=0.09,P=0.16;mean PCWP=(13.86±10.65)mmHg,R=0.16,P=0.03;mean CI=(2.26±0.56)L/min/m^2,R=0.08,P=0.20;mean SV=(51.79±13.70)ml/beat,R=-0.08,P=0.79;mean LVSWI=(30.62±14.16)g/(m·m^2),R=-0.10,P=0.93.Conclusion BNP had positive correlations with CVP、PAP、PCWP and CI.BNP had negative correlation with SV、LVSWI.
