大鼠甲状旁腺网膜内移植及二氧化碳气腹对移植物影响的实验研究

摘要 目的观察不同压力CO2气腹下,鼠网膜内甲状旁腺(PTG)移植物的功能与生存情况。方法采用同种、成年、雌性SD大鼠作为供、受体,建立去PTG模型受体。随机分对照组、低压气腹组、高压气腹组,每受体接受一供体的两个PTG,移植于网膜内。低压气腹组、高压气腹组移植术前、后分别向腹腔灌注1kPa、2kPa压力CO2气体各30min。比较移植前、后血钙和甲状旁腺素(PTH)值。结果对照组受体鼠移植后7d血钙值(2.41±0.10)mmol/L,低压气腹组为(2.47±0.05)mmol/L,高压气腹组为(2.44±0.09)mmol/L,与移植前比较,差异均有统计学意义(t分别=9.68、10.51、10.07,P均<0.05);PTH值分别升至(35.10±5.17)ng/L、(36.32±6.93)ng/L和(35.90±6.51)ng/L,与移植前比较,差异均有统计学意义(t分别=30.30、24.71、25.70,P均<0.05)。三组移植物存活期分别为:(29.75±7.97)d、(39.67±7.51)d和(41.41±8.15)d,两组气腹组与对照组比较,差异均有统计学意义(t分别=2.75、3.54,P均<0.05);两组气腹组之间比较,差异无统计学意义(t=0.54,P>0.05)。结论网膜内PTG移植能维持血钙正常水平;CO2气腹能延长网膜内PTG移植物存活期;正常范围不同压力CO2气腹不影响网膜内PTG移植物存活期。 Objective To explore the effects of pneumoperitoneum with carbon dioxide on parathyroid graft in the omentum of rat under different pressures. Methods Female, same breed, adult rats were involved as donor and receptor. Model rats were established with parathyroid insufficiency and divided into three groups at random: the contrast group (group Ⅰ), the lower pressure pneumoperitoneum group (group Ⅱ) and the higher pressure pneumoperitoneum group (group III). 2 parathyroid glands of 1 donor were transplanted into the omentum of a model rat. The rats in group Ⅱ and group Ⅲ were infused carbon dioxide into abdomen with 1 kPa and 2 kPa pressure for 30 mins before and after transplantation respectively. Serum calcium and parathyroid hormone were measured before and after transplantation. Results The average serum calcium 7 clays after transplantation were (2.41 ±0.10)mmol/L in group Ⅰ, (2.47 ±0.05)mmol/L in group Ⅱ, (2.44 ±0.09)mmol/L in group Ⅲ, which were significantly higher than those before transplantation (t=9.68,10.51,10.07, P〈0.05). Parathyroid hormone were (35.10±5.17)ng/L in group Ⅰ, (36.32±6.93)ng/L in group Ⅱ, (35.90±6.51)ng/L in groupⅢ, which were significantly higher than those before transplantation (t =30.30,24.71,25.70, P〈0.05). The survival period of graft were (29.75± 7.97)days in group Ⅰ, (39.67±7.51)days in group Ⅱ and (41.41±8.15)days in group Ⅲ. The survival period of graft in group II and III were significantly longer than that in group I (t = 2.75, 3.54, P〈0.05). Survival period of graft in group III had no significant difference compared with group II (t =0.54, P〉0.05).Conclusions FFG graft in omentum can maintain serum calcium levels. The pneumoperitoneum with carbon dioxide can prolong the survival period of parathyroid graft. Different pressures under normal scope of pneumoperitoneum do not influence the survival period of the parathyroid graft in omentum.
