盐酸法舒地尔对脑外伤后血浆内皮素变化的影响 被引量:6

摘要 目的观察盐酸法舒地尔对不同程度颅脑损伤后血浆内皮素(endothelin,ET)的活性变化的影响,探讨法舒地尔减轻颅脑损伤后继发性脑损害的可能性。方法本组50例颅脑损伤均在伤后24h内入院,重型颅脑损伤组15例:入院时GCS≤8分。轻中型颅脑损伤组35例:GCS>8分。患者按随机数字表法分为治疗组和对照组各25例。手术患者术后、非手术患者伤后48h开始给予盐酸法舒地尔注射液30mg/d,疗程10d,在给予盐酸法舒地尔注射液前、治疗后10d,对照组在治疗组对应时间采血,采用特异性放免法测定标本中ET的含量,所有病例临床随访3周,以治愈、残疾、死亡判断治疗结果。结果本组患者经法舒地尔治疗后,轻中型颅脑损伤和重型颅脑损伤ET水平分别为(52.41±7.03)、(78.21±7.78)ng/L,与治疗前(78.51±8.77)、(107.33±10.29)ng/L比较明显降低(P<0.05)。治疗后随访,轻中型颅脑损伤治疗组预后明显改善,重型颅脑损伤预后改善不明显。结论盐酸法舒地尔能够降低不同程度颅脑损伤血浆内皮素的活性,有利于改善脑缺血缺氧,但对重型颅脑损伤的临床预后作用有限。
