舒芬太尼对全麻患者气管插管时血流动力学、血浆肾上腺素、去甲肾上腺素的影响 被引量:7

摘要 目的:观察舒芬太尼对全麻患者气管插管时血流动力学、血浆肾上腺素、去甲肾上腺素的影响。方法:选择择期行全身麻醉手术的患者68例,随机分为两组,Ⅰ组(n=34)用舒芬太尼诱导,Ⅱ组(n=34)用芬太尼诱导。分别记录诱导前(T0)、诱导后(T1)、插管即刻(T2)、插管后1min(T3)、插管后3min(T4)、插管后5min(T5)各对应时点的收缩压(SBP)、舒张压(DBP)、心率(HR)以及T0、T1、T2、T4、T5对应时点的血浆肾上腺素(E)、去甲肾上腺素(NE)数值。结果:两组的SBP、DBP、HRT1与T0比较均有明显下降,差异有特别显著性(P<0.01)。Ⅰ组SBP、DBP、HR在T2、T3、T4、T5与T0比较无明显变化(P>0.05)。Ⅱ组SBP、DBP、HR在T2、T3、T4与T0比较有大幅上升,与Ⅰ组比较亦有大幅上升,差异有显著性(P<0.01或P<0.05)。Ⅰ组患者T2、T4、T5与T0比较E、NE差异不明显(P>0.05);Ⅱ组患者T2、T4、T5较T0均明显升高,且显著高于Ⅰ组,差异有显著性(P<0.01或P<0.05)。结论:舒芬太尼在全麻患者气管插管时血流动力学稳定,对血浆肾上腺素、去甲肾上腺素的影响小,能有效的抑制气管插管时引起的应激反应。 Objective Observation the effects of Sufentanil on hemodynamics and Epinephriine (E)and Norpinephine (NE)during induction of General anesthesia.Methods Sixty-eight patients were randomized to two groups:Sufentanil group (group I n=34) and fentanyl group (group Ⅱ n=34), systolic pressure(SBP), diastolic pressure (DBP),heart rate (HR) were recorded respectively at the times of before anesthesia(T0), atter anesthesia(T0, intubation(T2), 1 min after intubation(T3), 3 rain after intubation (T4), 5 min after intabation (T5). E, NE were recorded respectively at the times of To, T2, T4, T5. Results SBP, DBP and HR were significantly lower in all patients at T1 compared with To. SBP, DBP, HR ofgroup I were shown no significant difference during T2,T3,T4,T5 compared with To. SBP, DBP, HR of)group II were significantly higher during T2, T3, T4,Tscompared with T0,and significantly higher compared with group I too.E, NE of group I were shown no significant difference during T2,T4,T5 compared with T0.E, NE of group Ⅱ were significantly higher during T2,T4 compared with To, and significantly higher compared with group I too.Conclusion The values of hemodynamics and Epinephriine and Norpinephine will not be effected during the process of the induction of general anesthesia with sufentanil. And can effectively inhibits the stress response during intubation.
