小儿肾病综合征患者血清胆固醇酯转运蛋白浓度变化及与高脂血症的关系 被引量:1

摘要 目的检测原发性小儿肾病综合征(NS)患者血清胆固醇酯转运蛋白(CETP)浓度,分析CETP水平与高脂血症的关系及其在小儿NS脂质紊乱发生中的意义。方法用ELISA法测定113例患儿血清CETP浓度,同时测定肾功能和血脂水平,以年龄?性别匹配的194例健康儿童为正常对照。结果NS患儿血脂和脂蛋白浓度明显高于正常儿童。患儿血清CETP浓度显著高于对照组[(5.71±3.10)mg/L vs(3.61±2.28)mg/L,P<0.001],且与血脂和脂蛋白浓度呈强正相关关系(P<0.0001)。经Logistic多元回归校正血、尿蛋白含量等因素后,所有血脂及脂蛋白水平不再与CETP显著相关。结论NS患儿血清CETP水平明显升高,可能是高脂血症形成的原因之一。血清CETP水平的测定有助于NS病情和预后判断。 Objective To examine the alteration of cholesteryl ester transfer protein (CETP) level in serum of children with idiopathic nephrotic syndrome (NS) and investigate the relationship of CETP with hyperlipidemia and the role of CETP in childhood NS. Methods The concen-trations of CETP were determined in serum from 113 children with idiopathic NS and 194 control subjects matched for age and sex by ELISA. The parameters for renal function and lipid traits in serum were simuhaneously measured in the both groups. Results NS children presented significantly higher levels of lipids and lipoproteins than controls. CETP concentrations in patients were significantly increased (5.71±3.10 mg/L vs. 3.61±2.28 mg/L, P〈0.001), and exhibited strong positive Correlations with lipid traits. However, following the calibration of serum albumin and proteinuria by logistic multiple linear regression no significant relations of CETP concentrations with the variation of lipid and lipoprotein were found in NS patients. Conclusion CETP concentrations are markedly elevated in NS children, which may play an impor- tant role in hyperlipidemia. Serum CETP level is useful for evaluating the progression and prognosis of childhood NS.
