摘要 目的测定并分析T细胞亚群和免疫球蛋白在不同类型过敏性紫癜(Henoch-Schonlein purpura,HSP)急性期患儿外周血中的变化趋势及其在HSP发病机制中的作用。方法选取HSP患儿114例,符合美国风湿病协会的诊断分类标准,2周内未使用糖皮质激素及其他免疫抑制剂的初发病例,按临床表现类型分为5组:皮肤型组29例,关节型组26例,腹型组19例,肾型组13例,混合型组为27例。正常儿童作为对照组29例。CD3、CD4及CD8等T细胞亚群百分率用流式细胞仪检查;IgA、IgG、IgM及补体C3、C4含量用特定蛋白分析系统检查。结果在不同类型HSP患儿急性期,T细胞亚群CD4百分率降低,CD8百分率增高,CD4/CD8比值降低(P<0.05)。B细胞除皮肤型组外其他各组与对照组相比均增高(P<0.05);自然杀伤细胞在腹型和肾型组较对照组减低(P<0.05)。与对照组相比,IgA在混合型组中增高,IgM在关节型和混合型组减低(P<0.05)。结论HSP患儿急性期存在T细胞亚群功能紊乱,免疫球蛋白分泌异常,IgA外周血中水平高低可能与多种因素有关。 Objective To observe the changes of T cell subsets and immunoglobulin in blood of different subtypes of Henoch-Schon- lein purpura(HSP)in the acute phase,and to explore their roles in the pathogenesis of HSP. Methods One hundred and fourteen children diagnosed as HSP according to classification criteria of American Rheumatism Association were chosen as subjects, untreated with steroids and immune depressants in 2 weeks. They were divided into 5 groups according to clinical manifestations: skin group( n = 29) ,joint group( n = 26), abdomen group( n = 19), kidney group ( n - 13), mixed group ( n = 27). Twenty nine normal children were chosen as controls. The levels of CD3, CD4 and CD8 were detected by flow cytometry. IgA, IgG, IgM,C3 and C4 were detected by specific protein analytical system. Results Compared with controls, the level of CD4 and the ratio of CD4/CD8 in children with HSP significantly decreased( P (0.05), while the level of CD8 increased( P 〈 0.05 ). Except that in skin group, the levels of B cells in the other 4 groups were significantly higher than in control group(P 〈 0.05 ). NK cells were significantly lower in abdomen group and kidney group than in control group(P〈 0.05). IgA were significantly higher in mixed group than in control group(P〈0.05). However, IgM in joint group and mixed group were significantly lower than in control group(P 〈 O. 05 ). Conclusion There is disorders of T cell subsets and parasecretion of immunoglobulin in acute HSP. The levels of blood IgA in HSP might be related to multiple factors.
