摘要
[目的]探讨Midas Rex高速磨钻结合脊柱显微内窥镜系统治疗腰椎管狭窄症的疗效。[方法]采用脊柱后路显微内窥镜系统和Midas Rex高速磨钻对45例腰椎管狭窄症患者进行手术治疗,其中中央椎管狭窄14例,脊神经根管狭窄13例,混合型狭窄18例,在显微内窥镜下Midas Rex高速磨钻进行椎管减压。[结果]平均手术时间70min,平均出血量50ml,平均住院7d,术后d2可下地活动,并行腰背肌锻炼,平均随访13个月,总有效率97.8%,优良率86.7%。1例咬骨钳咬除黄韧带时致硬脊膜破裂,无神经损伤。[结论]Midas Rex高速磨钻能在内窥镜下对任何部位骨质进行磨削,对椎管进行环形减压,减压充分彻底,手术创伤小,疗效满意。
[Objective] To explore the clinical effects of Midas Rex high-speed drill combined with micro-endoscopic system in the lumbar spinal stenosis surgery. [ Methods] The posterior micro-endoscopic system combined with Midas Rex high-speed drill system were used in 45 cases with minimally invasive lumbar spinal stenosis operation. 14 cases of spinal canal stenosis, 13 cases of spinal nerve root canal stenosis and 18 cases of mixed types were treated by spinal canal decompression using micro-endoscopic system combined with Midas Rex high-speed drill system. [ Results] The average surgery time was 70 minutes. The average blood lose was 50ml and average hospital day was 7 days.After operation, the patients were allowed to walk and do waist musculi dorsi exercise in two days. The average follow up time was thirteen months. The total improvement rate was 97.8% and the fineness rate was 86.7%. One case of spinal dura mater was teared, when using gouge forceps to remove arcuate ligaments, with no nerve injury, was noted. [ Conclusion] Midas Rex high-speed drill can grind and cut any position bone when combined with micro-endoscopic system, it can do ring-shaped decompression of spinal canal completely and accurately. It is minimally invasive spinal surgery and has satisfying clinic effects.
