摘要
目的探讨单一使用地塞米松、西米替丁及两药联合使用预防化疗性静脉炎的效果。方法将40只免随机分为盖诺组、西米替丁组、地塞米松组和地塞米松与西米替丁联合组,每组各20只兔耳。用药量:盖诺为1.93mg/kg,西米替丁为9.63mg/kg,地塞米松为0.24mg/kg。用药方法:盖诺组直接静脉注射盖诺;西米替丁组先静脉注射西米替丁,再静脉注射盖诺;地塞米松组先静脉注射地塞米松,再静脉注射盖诺;联合组先静脉注射地塞米松和西米替丁后,再静脉注射盖诺。结果肉眼观:地塞米松组、西米替丁组及联合组静脉炎发生率显著低于盖诺组(OR值分别为0.23,0.16,0.07);地塞米松组与西米替丁组比较,差异无显著性意义(P>0.05);联合组与地塞米松组加西米替丁组比较,差异无显著性意义(P>0.05)。镜下观:地塞米松组、西米替丁组及联合组静脉病理损伤程度显著轻于盖诺组(OR值分别为0.08,0.07,0.03);地塞米松组与西米替丁组比较,差异无显著性意义(P>0.05);联合组与地塞米松组加西米替丁组比较,差异无显著性意义(P>0.05)。两药无相乘交互作用,但根据OR值判断,两药有相加交互作用。结论地塞米松、西米替丁均有预防盖诺所致静脉炎的作用,联合用药效果优于单一用药。
Objective To evaluate the preventive effect of cimetidine, dexamethasone, and combination use for chemotherapy-induced phlebitis. Methods Forty Zealand rabbits were randomized into four groups equally: a vinorelbine group, a cimetidine group, a dexamethason group, and a cimetidine combined with dexamethasone group (the combined group) ; the dosage of vinorelbine, cimetidine, dexamethason was 1.93 mg/kg, 9.63 mg/kg and 0.24 mg/kg respectively. In the vinorelbine group, vinorelbine was given intravenously directly, while in the other groups, the vinorelbine was administered following infusion of preventive drug. Results By naked eye, the incidence of phlebitis was significantly decreased in the eimetidine group, dexamethason group and the combined group (the OR values were 0.23, 0.16 and 0.07 respectively). Microscopically, the severity of tissue damage was significantly ameliorated in the cimetidine group, dexamethason group and the combined group (the OR values were 0.08, 0.07 and 0.03 respectively). There were no significant differences in occurence rate and the severity of tiscue damage between the cimetidine group and the dexamethason group (P〉0.05 for all), and among the combined group, the cimetidine group plus the dexamethason group as well (P〉0.05 for all), which indicated there was no multiplication interaction between cimetidine and dexamethasone, but the OR values indicated these two drugs possessed the sum interaction. Conclusion Cimetidine and dexamethasone can prevent phlebitis induced by vinorelbine; the combined regimen may achieve cooperative prophylactic effect.
