自体血液回收技术对红细胞形态及功能的影响 被引量:7

摘要 目的研究应用自体血液回收机(Cell Saver)对红细胞形态及功能的影响。方法选择20例非体外循环下冠状动脉搭桥术应用Cell Saver的患者作为研究对象。用新型激光衍射法测定所有患者术前、回收血、洗涤血以及回输洗涤血3 h后体内红细胞的变形能力,包括红细胞最大变形指数、红细胞综合变形指数、红细胞取向指数和红细胞小变形指数。用扫描电镜观察各时间点红细胞的形态变化。结果红细胞最大变形指数各时间点无显著差异(P>0.05);红细胞综合变形指数、红细胞取向指数和红细胞小变形指数变化趋势一致,洗涤后和回输后明显高于洗涤前(P<0.05)。回收血液中的红细胞表现为满视野棘状细胞,细胞表面有许多突起出现;洗涤后,红细胞形态与洗涤前比较异形结构有所恢复,部分红细胞接近正常双凹圆盘状。结论自体血液回收技术对红细胞的变形能力及形态无明显的不利影响。 OBJECTIVE To observe the effect of Cell Saver on morphology and function of erythrocytes perioperatively.METHODS 20 patients who underwent off-pump coronary artery bypass grafting(OPCABG) were included in this study.Cell Saver was adopted for the blood conservation.Blood samples from patients were drawn before operation and 3 hours after autotransfusion.Unprocessed blood and processed blood in cell saver were also measured.The deformability of erythrocytes was measured with ektacytometry which included maximum deformability,integrate deformation index,small deformation index and orientation index.The morphology of erythrocytes was measured with scanning electron microscope.RESULTS The maximum deformability of erythrocytes in unprocessed blood had a nonsignificant decline and restored after processing by cell-saver(P〉0.05).The deformability including integrate deformation index,small deformation index and orientation index had similar trends.These parameters of erythrocytes in unprocessed blood decreased and restored in processed blood(P〈0.05).The morphological examination indicated that the suction used in autotransfusion can result in echinocytic shape transformation and most of the transformation was reversible after the process of centrifuging and washing.CONCLUSION Autotransfusion has no obvious adverse effect on the deformability and function of erythrocytes.
