Relation between Serum Lipoprotein(a) and Severity of Coronary Artery Lesion and Left Ventricular Performance in Survivors of Acute Myocardial Infarction

摘要 目的探讨急性心肌梗死(AMI)患者血浆脂蛋白(a)[Lp(a)]与冠脉病变及左室心功能的关系。方法102例AMI患者按血浆Lp(a)浓度分为高Lp(a)组(≥300mg/L)及低Lp(a)组(<300mg/L),行冠脉造影及超声心动图检查,比较两组冠脉病变特点及心功能的差异。结果高Lp(a)组梗死相关血管(IRA)的残余狭窄程度、血管闭塞率、冠脉狭窄分数及室壁运动记分指数均明显高于低Lp(a)组(P<0.05),其IRA的心肌梗死溶栓治疗试验(TIMI)血流评分、射血分数均明显低于低Lp(a)组(P<0.05)。结论血浆Lp(a)异常升高的AMI患者,其IRA的残余狭窄程度加重,血流速度缓慢,血管闭塞率增加,导致室壁运动异常加重,射血分数下降,心功能恶化。 Objective To investigate the influence of Lp(a) on the coronary artery lesion and left ventricular performance in acute myocardial infarction (AMI) patients. Methods We studied 102 AMI patients who underwent coronary angiography and echocardiography. Morning fasting blood was collected. Serum lipid and blood sugar were measured. The results were compared between the low Lp(a) group (〈 300 mg/L) and the high Lp(a) group (≥300 mg/L). Results The severity of coronary artery atherosclerosls and the residual stenosis of the infarct - related artery were higher in the high Lp(a) group compared to the low Lp(a) group (P〈0.05). Antegrade TIMI flow in the infarct-related artery was lower in the high Lp(a) group (P〈0. 05). The number of occluded IRA was higher in patients with high Lp(a) levels (P〈0.05). Results wall motion score index was higher and global ejection fraction was lower in the high Lp(a) group (P〈0. 05). Conclusion Elevated Lp(a) levels affected the residual stenosis, antegrade TIMI flow the number of occluded IRA and the severity of coronary artery atherosclerosis in AMI patients. These led to a greater extent of abnormal wall motion and the decline of ejection fraction, thus impaired left ventricular function.
