摘要
报告体外循环（ＣＰＢ）术后急性呼吸衰竭３０例的救治体会，提出预防要点。认为：使用先进的ＣＰＢ设备、缩短ＣＰＢ时间、使用微栓过滤器、做好术后呼吸管理等可减少肺部并发症的发生。依据临床表现及时诊断、及时行气管插管或切开、正确的机械辅助呼吸方式是救治成功的关键。应用呼气末正压通气、东莨菪碱及有效地控制感染有利于提高疗效。
Management and prevention measures of 30 cases of acute respiratory failure after cardiopulmonary bypass(CPB) surgery were summarized. The authors suggest that application of advanced CPB equipment, shortening CPB time, using microembolism filter and caring respiration well after operation can reduce pulmonary complications. The key points of successful management are to diagnose promptly, to perform tracheal cannula or tracheotomy in time, to chose proper respiratory mood. Application of PEEP and hyoscine, controlling infection effectively will improve curative effect.
