缺氧缺血性脑病新生儿免疫功能变化的临床意义 被引量:5

摘要 目的检测缺氧缺血性脑病(HIE)新生儿血清IgG、IgA、IgM及C3、C4的变化,了解HIE对免疫功能的影响。方法HIE患儿172例,根据年龄、病情轻重分为1~3天组84例,其中轻度HIE36例,中重度HIE48例;4~7天组88例,其中轻度HIE41例,中重度HIE47例。对照组86例为同期住院非HIE足月新生儿。检测血清IgG、IgA、IgM及C3、C4,相关结果进行统计学分析。结果1—3天组和4—7天组轻度HIE及中重度HIE患儿血清IgM、C3、C4均低于对照组(P〈0.05),轻度HIE及中重度HIE患儿血清IgG、IgA与对照组差异均无统计学意义(P〉0.05);1~3天组和4~7天组轻度HIE及中重度HIE患儿间血清IgG、IgA、IgM、C3、C4差异均无统计学意义(P〉0.05)。结论HIE可使患儿IgM及补体水平降低,但与HIE的程度无关。 Objective To investigate tbe changes of serum IgG, IgA, IgM, C3 and C4 in neonates with hypoxic - ischemic encephalopathy (HIE). Methods 172 neonates with HIE and 86 non - HIE normal full - term infants were recruited in the study. The HIE neonates were divided into a 1 - 3 day group ( n = 84) and a 4 - 6 day group ( n = 88), which were further divided into mild and moderate - severe subgroups. The Igs and complements in serum were determined by radial immunodiffusion assay. Results The levels of IgM, C3 and Ca were lower in both 1 - 3 day and 4 - 6 day groups than in the control (P 〈 0.05 ). But the levels of IgG and IgA did not show such differences significantly (P 〉 0.05 ). No significant differences in the results were found between the two age groups of the babies, or the varied severities of HIE, in the varied Igs and complements ( P 〉 0. 05). Conclusion The levels of IgM, C3 and C4 are lowered in HIE neonstes, but the severity of HIE does not contribute to these changes.
