高血压胰岛素抵抗与超敏C-反应蛋白和纤维蛋白的关系分析 被引量:2

摘要 目的探讨高血压患者胰岛素抵抗与超敏C-反应蛋白(hsCRP)及纤维蛋白(Fg)的关系。方法选择65例原发性高血压患者,用胰岛素抵抗指数(IRI)作为胰岛素敏感性评价指标,把单纯高血压患者分为高血压无胰岛素抵抗组(A组,21例)和高血压胰岛素抵抗组(B组,20例),另高血压合并2型糖尿病组(c组,24例),采集空腹静脉血,测定3组患者hsCRP以及Fg、血糖、胰岛素、血脂和载脂蛋白。结果B组及c组hsCRP和Fg水平高于A组(p<0.05)。Logistic回归分析显示:IRI与Fg和hsCRP呈正相关(OR分别是2.901,1.318,均p<0.01)。结论hsCRP和Fg反映出机体炎症活性,在原发性高血压的胰岛素抵抗及2型糖尿病发病机制中可能起一定作用。 Objective To study on relation of insulin resistance with hyper-sensitivity C-reaction protein (hsCRP) and fibrinogen (Fg) in the patients with essential hypertension. Methods 65 cases of EH were selected, IR1 was applied to estimate insulin resistance, essential hypertension patients without type 2 diabetes mellitus were divided into non IR group(group A. n =21)and IR group(group B,rt =20) o 24 cases of EH with T2 DM were group Co hsCRP, Fg, fasting glucose, fasting insulin, blood lipid and lipoprotein were measured after 12 h over night fast. Resuits The serum levels of hsCRP and Fg in group B and C were Mgher than those in group A (p〈0.05). Logistic regression analysis showed that IR1 was positively correlated with Fg and hsCRP(OR=2. 901 and 1. 318,both p〈0.01). Conclusion hsCRP and Fg may be useful markers of systemic inflammation. The serum levels may play a role in the pathogenesis of essential hypertension with insulin resistance and type 2 diabetes mellitus.
