摘要
目的:探讨64层螺旋CT血管造影(CTA)诊断下肢动脉疾病的临床价值。材料和方法:对125例临床疑有下肢动脉疾病的患者行CTA检查。扫描范围1000~1500mm,层厚1.25mm,螺距0.984。造影剂剂用量90~100ml,注射速度3ml/s,高压注射器自动触发扫描,扫描时间20—30S,对全部图像进行MIP、VRT、MPR重建使下肢血管清晰显示,每例患者血管分段为10段,并对图像进行评价。其中有20例患者1周内行血管造影检查,并进行相同血管节段对照研究。结果:CTA结果与DSA结果有较好的一致性,阳性预测率92.5%,阴性预测率98.6%。结论:64层CTA能清晰地显示下肢动脉及其病变,可以成为糖尿病下肢动脉疾病术前评价和筛选的主要方法。
Purpose: To evaluate the clinical application of 64-slice spiral CT angiography in the lower extremity arterial disease of diabetes. Materials and Methods: 125 patients with arterial disease of the lower extremities of diabetes were submitted to 64-Slice spiral CTA.Scanner with range in 1000 - 1500 mm,slice thickness 1.25 mm,pitch 0. 984. The volume of contrast agent was 90 - 100 ml,flow rate 3 ml/ s, scan was started with smartprep auto-trigger. All reconstructed images were reformed by means of MIP ,VR,MPR. The vessels in every cases segements wese divided into 10 segements,whieh were evaluated. 20cases underwent DSA in one weeks. The accuracy of 64-slice spiral CT angiography and conventional digital subtraction angiography was compared. Results: Compared with conventional DSA, the sensitivity and specificity of 64 - slice spiral CT angiography with regard to detection of hemodynamically significant stenosis in all 200 arterial segments were 98.6% and 92.5%, respectively. Conclusion: The arterial lesions in the lower extremity of diabetes can be well demonstrated by 64-slice spiral CTA , which is a good method for pre-surgery evaluation and screening in the lower extremities of diabetes.
