摘要
目的分析鼻窦炎由急性向慢性转变过程中的病理及细菌学变化。方法用浸有金黄色葡萄球菌的膨胀海绵条阻塞鼻腔建立鼻源性鼻窦炎动物模型,分别于实验后1、2、4、6、8、10周时内镜下观察上颌窦,取出窦腔黏膜标本分别进行细菌学培养及组织病理学检查。结果早期黏膜以单核细胞浸润为主;2周后可见淋巴细胞、中性粒细胞浸润,杯状细胞增加、腺体增生;4~6周后大量的杯状细胞生成;8周以后窦腔黏膜水肿严重,黏膜增厚。在整个过程中,杯状细胞增加最明显。早期窦内细菌学培养主要为金黄色葡萄球菌,以后逐渐被肺炎球菌、莫拉杆菌、大肠杆菌所代替。结论通过鼻腔诱导引起的鼻源性鼻窦炎其黏膜所表现的病理变化经过了四个连续的病理阶段,客观反映了鼻窦炎由急性向慢性转变过程中的病理变化,随着病程的发展,机会致病菌的感染逐渐增多。
Objective To investigate the pathological and bacteriologic changes of rhinogenie sinusitis in rabbits. Methods An animal model of rhinogenic sinusitis was established by implanting an expansive sponge soaked with staphylococcus aureus into the rabbit' s nasal cavity. The animals were sacrificed at specific time points. The mucosa were carefully examined for evidence of inflammatory changes with histopathological and bacteriologic methods. Results At the first week, monocyte was the predominant cell in the mucosa. At the second week, lymphocyte, neutrophil appeared with increase of goblet ceils and glands. At the fourth and sixth week, goblet cells became abundant. After the eighth week, the mucosa got seriously oedema and incrassated. During the whole course, goblet cells increased obviously. The germicuhure showed staphylococcus aureus at the early stage, which was gradually replaced by pneumoniae, Moraxelle catarrhalis, and Escberiehia coli. Conclusion A model of rhinogenic sinusitis is successfully established by nasal cavity packing. Four continuous stages arc included in the pathological changes of sinusal mucosa, which may reveal the pathological course of sinusitis from acute stage to chronic. The cultivate rate of opportunistical pathogenic bacteria is related to the process of sinusitis.
