摘要
目的研究钙调酶抑制剂环孢素(CsA)与他克莫司(tacrolimus)对肾移植术后患者细胞因子IL-2,IL-4蛋白表达的影响。方法分离血浆,利用双抗体夹心ABC-ELISA法测定患者血浆中细胞因子IL-2和IL-4的蛋白表达量。结果CsA服药前后可明显降低IL-2的蛋白表达水平[(263.43±64.50)和(228.99±78.84 ng.L-1],明显增加IL-4的蛋白表达水平[(54.78±17.40)和(61.44±13.22)ng.L-1(P<0.05)];他克莫司对IL-2[(162.79±60.56)和(164.59±68.66)ng.L-1]和IL-4[(50.73±7.07)和(53.36±9.17)ng.L-1]无明显影响(P>0.05),但他克莫司对细胞因子IL-2与IL-4的抑制能力较CsA强,且服用CsA的个体差异比他克莫司明显;两用药组血药浓度与IL-2,IL-4均无相关性。结论CsA和他克莫司可以通过同时影响细胞因子IL-2和IL-4的蛋白表达发挥免疫抑制作用,而且他克莫司比CsA能更好地抑制肾移植患者的机体免疫,安全性也较CsA好。
OBJECTIVE To investigate the effects and mechanisms of cyclosporine and tacrolimus on the protein expression of IL-2 and IL-4 in renal transplant recipients.METHODS The plasma was isolated from whole blood of patients.Double antigen sandwich ABC-ELISA was performed to detect the production of IL-2 and IL-4.RESULTS CsA inhibited the content of IL-2[(263.43±64.50) vs(228.99±78.84)ng·L-1],but promoted that of IL-4[(54.78±17.40) vs(61.44±13.22) ng·L-1] in plasma(P〈0.05).The effect of tacrolimus on IL-2 [(162.79±60.56) vs(164.59±68.66) ng·L-1] and IL-4 [(50.73±7.07) vs(53.36±9.17) ng·L-1] was not as conspicuous as CsA group(P〉0.05),while the inhibition of tacrolimus on IL-2 and IL-4 was stronger than that of CsA.Furthermore,the individual variation on IL-2 and IL-4 of CsA was more significant than that of tacrolimus.There was no significant correlation between IL-2 or IL-4 and plasma drug concentration in both groups.CONCLUSION Our results suggest that CsA and tacrolimus exert their effects on immune suppression by influencing the expression of IL-2 and IL-4.The individual variation in patients undergoing CsA has significant difference,so tacrolimus is much safer than CsA.
