Nutritional evaluation and related factors in hospitalized patients with systemic lupus erythematosus

摘要 目的:了解系统性红斑狼疮(SLE)住院病人营养状况及其影响因素。方法:对148例SLE住院病人进行营养调查,测身高、体质量,并计算体质量指数(BMI)。病人入院时,空腹采静脉血检测Hb、ALB。根据病史采集激素累积量和使用时间,并计算激素平均量。对所有使用商品化EN制剂、复方氨基酸、脂肪乳剂的病人,均作为接受营养干预。结果:148例病人中BMI异常者占46.5%,其中BMI降低者占31.8%,BMI增高者占14.7%,低蛋白血症占53.9%、贫血占58.3%。其中中、重度BMI降低、低蛋白血症、贫血占营养不良病人的比例,分别为41.4%、52.6%和44.0%。激素冲击对ALB和Hb有明显影响,对BMI作用不明显。激素累积量和平均量均与ALB成负相关(r=-0.056,-0.072;P<0.05)。148例病人营养干预率仅为9.0%。结论:SLE住院病人中,BMI异常、贫血、低蛋白血症的发生率高,但营养干预率低。年龄、病程和激素治疗,均可影响病人的营养状况。使用ALB评价SLE病人的营养状况更为合理可靠。 Objective: To temic lupus erythematosus. investigate nutrition status and related factors in hospitalized patients with sysselected from Renji hospital. Methods: 148 hospitalized patients with systemic lupus erythematosus were Anthropometric measurements and laboratory tests were carried out to evaluate nutritional status according to BMI, Hb and ALB. Accumulation and average dose of glucocorticoid therapy were collected based on the medical history. Patients who received any kind of enteral or parenteral nutritional products were defined as nutritional intervention. Results: The rate of abnormal BMI, hypoalbuminemia and anemia were 46.5%, 53.9% and 58.3% respectively in hospitalized patients with systemic lupus erythematosus. Among the malnourished patients, the prevalence of medium or serious malnutrition was 41.4% to 52.6%. Age was showed little influence on BMI,Hb and ALB. Glucocorticoid therapy affected the level of Hb and ALB significantly. Furthermore, the negative relationship was found between the accumulation dose , average dose of glucocorticoid therapy and the level of ALB ( r = - 0. 056, - 0. 072 ;P 〈 0.05 ). It was only 9.0% who received nutrition intervention in 148 patients. Conclusion: The malnutrition is very common in hospitalized patients with systemic lupus erythematosus, while rate of nutritional intervention was low in patients with systemic lupus erythematosus . Course of disease and glucocorticoid therapy have important impacts on the nutrition status of hospitalized patients with systemic lupus erythematosus. ALB is better as nutritional indicators than BMI and Hb patients with SLE.
