摘要
目的研究激光融脂治疗局部脂肪堆积的临床价值。方法使用Nd:YAG激光经光纤导入皮下脂肪层,根据治疗部位精确治疗参数(波长1064 nm,单脉冲能量150 mJ,频率40 Hz)后,于脂肪堆积部位的不同层次行扇形照射,在脂肪融解后将其挤出或吸出。并观察脂肪组织的组织学变化及患者的术后反应和临床效果。自2005年11月至2008年5月,对35例面颈部及上臂局部脂肪堆积的患者进行了激光融脂术。结果术后随访35例患者1～20个月。与术前比较,局部臃肿明显改善,外形良好;B超检查脂肪厚度明显减少;无血肿、局部麻木、感染、局部硬结等并发症发生,术区无明显瘢痕。融脂后的吸出混合物中,血液成分明显少于传统的负压吸脂术。结论Nd:YAG激光具有确切的脂肪融解作用,对治疗局部脂肪堆积有优于传统负压吸脂法的临床应用价值。
Objective To study the clinical application of laser lipolysis for treatment of localized adiposis. Methods Nd： YAG laser was applied to adipose tissues layers by use of optic fiber. The zone of application was fan - shaped with precise treatment parameters which were determined by the locations （wavelength： 1064 nm, energy per pulse： 150 mJ, frequency： 40 Hz）, and this technique was carried out in different layers of local adiposis. The histologic changes in treated adipose tissues, post - operative reactions and clinical results were studied. From Nov 2005 to May 2008, 35 patients with localized adiposis were treated by laser lipolysis. Results All patients were followed for 1 to 20 months. Compared to the preoperative condition, the thickness of the fat tissue was decreased. Moreover, there was no hematoma formation, numbness, infection, local induration or other complications. The scars form the procedure were not obvious, and bleeding was less than that caused by conventional liposuction. Conclusion Nd： YAG laser has a definite lipolytic effect on adipose tissue, and is a more effective approach for the treatment of adiposis than conventional liposuction.
