摘要
目的为了解我院神经外科医院感染发病特点及相关危险因素,探讨有效的预防控制措施。方法对2005年1月-2007年12月神经外科医院感染的病例进行回顾性调查分析。结果神经外科医院例次感染率为7.39%,与全院同期医院例次感染率2.92%比较,有高度显著性差异(P<0.01),感染部位以下呼吸道感染居首位(56.64%),其次是上呼吸道及皮肤软组织感染等;呼吸机、气管切开、动静脉插管、尿管插管等侵入性操作是医院感染的高危因素,病原菌以G-杆菌为主,占57.94%,其次G+杆菌占38.32%,真菌占3.74%。结论应将神经外科患者作为医院感染重点监控对象,加强无菌技术操作规程;提高医务人员洗手率;尽量避免或减少侵入性操作;加强基础护理及专科护理和抗菌素的管理等是控制神经外科医院感染的有效措施。
Objective To find valid prevention and management measures to control spread of infectious agents in neurosurgery department of Shatou Hospital. We investigated all the traits and factors that may cause the neurosurgery patients being infected during the hospital stay. Methods By reviewing the casebooks between January 2005 and December 2007 in neurosurgery department, the infected patients' symptoms and characteristics were analyzed. Results The data in the casebooks clearly identified that the infected patients in neurosurgery department were significantly higher (.7.39%) than the overall hospital infection rate ( 2.92% ) in Shatou hospital during the same time period, there were statistically significant differences between them [ P〈 0. Ol ). Most cases got lower respiratory infection (56.64 % ). others were upper respiratory infection and skin and soft tissue infections. These patients suffered and got infected with infectious agent were due to the careless operation of surgery and medical treatment, such as respirator, tracheotomy, arteriovenous intubatip and catheter. Pathogenic bacteria were mainly Gbacillus ( 57.94 % ) , followed by G^- bacteria ( 38.32 % ) and fugi ( 3.74 % ). Conclusions The results suggest that neurosurgery department slqould be listed as prior monitory unit. Enhancing anti infectious skill and operation procedure, encouraging professionals' hands washing, reducing incursive operations, increasing basic care and special care, and strengthening management are important to control nosooomial infection in neurosurgery ward.
