三维适形放疗联合甘氨双唑钠治疗复发鼻咽癌的疗效观察

摘要 目的观察甘氨双唑钠(CMNa)联合三维适形放疗(3DCRT)对局部复发鼻咽癌的放射增放作用。方法46例复发者均为首程放疗后达基本痊愈或明显缓解,一段时间(〉6个月)后又出现鼻塞、头痛、涕血等情况恶化的临床症状,经病理检查确诊为复发,随机分为研究组和对照组,每组23例。研究组CMNa 800mg/m^2用生理盐水100ml稀释后静脉输入,输入结束后60min内放疗,放疗均采用西门子直线加速器,6mV-X线,Brain LAB公司三维治疗计划系统行三维适形放疗(3DCRT)。2~4Gy/次,4~5次/周。照射剂量研究组50~55Gy、对照组60~65Gy。采用彩超、SCT和MRI比较两组近期疗效,并比较两组毒副作用差别。结果研究组近期有效率(CR+PR)74%,对照为44%(P〈0.05)。两组毒副作用发生率中白细胞下降,Ⅲ度以上口腔黏膜反应,脑水肿症状分别为22%(5/23)、0(0/23)、30%(7/23)和17%(4/23)、22%(5/23)、22%(5/23)(P〉0.05)。结论CMNa可增加局部复发鼻咽癌的放射敏感性,表现为在降低放疗剂量的情况下,提高了复发鼻咽癌的近期疗效且不增加毒副作用。 Objective To investigate the effect of sodium glycid idazole(CMNa) in re - radiotheraphy aginst locally recurrent nasopharyngeal carcinoma after radical radiotheraphy. Methods Forty - six were randomly divided into the study group (23 patients) and control group (also 23 patients). The patients in study group were treated by intravenous infusion of CMNA 800 mg/m2 diluted in 100 ml sodium chloride. Radiotheraphy were given 60 min 'after each infusion. All patients received three dimensional conformal radiotheraphy (3DCRT) at :2 -4 Gy per fraction,4 -5 fraction a week to a total dose of 50 -55 Gy in study group and 60 - 65 Gy in control group. The short - terms therapeutic effect and side effects were compared between the two groups with SCT and MRI. Results The short - term effective rates ( CR + PR) were 74% in the study group and 44% in the control group (P〈0.05), There were no significant differences in the side effects between the two groups (P〉0.05). Conclusion CMNa may enhance the radiosensitivity of locally recurrent nasopharyngeal carcinoma cells, 3DCRT combined with sodium glycididazole may improve the short-term effect rate, reduce the radiotheraphy dose and does not increase the side effects.
