甘氨双唑钠在Ⅲ期非小细胞肺癌三维适形放疗中应用的观察 被引量:5

摘要 目的:观察甘氨双唑钠(CMNa)对Ⅲ期非小细胞肺癌(NSCLC)三维适形放疗(3-DCRT)的放射增敏作用及其安全性。方法:经病理和细胞学确诊的Ⅲ期NSCLC患者120例,采用随机双盲对照法分为A组(放疗+CMNa组)45例和B组(放疗+安慰剂组)45例。A组用CMNa1000mg/次,3次/周(星期一、三、五用药);CMNa溶于100mL生理盐水中,30min内滴完;B组用安慰剂加生理盐水,方法同A组,直至疗程结束。输液结束后30~60min内两组进行3-DCRT,每周5次,每次2Gy,总剂量为60~70Gy。结果:A组CR率和总有效率(CR+PR)均明显高于B组;A组患者治疗达到PR和CR时的平均照射剂量均低于B组;两组患者不良反应发生率的比较差异无统计学意义,P=0.54。结论:CMNa对Ⅲ期NSCLC3-DCRT有较肯定的放射增敏作用,明显提高了NSCLC的近期疗效,不增加不良反应。 OBJECTIVE: To observe radiosentization and security of three-dimensional confonnal radiotherapy (3-DCRT)to non-small cell lung cancer(NSCLC) stage Ⅲ with glycididazole natrium (CMNa). METHODS: A total of 120 cases of NSCI.C in stage Ⅲ were divided into Group A (45 cases, radiotherapy+CMNa)and Group B (45 cases, radiotherapy + placebo) randomly. CMNa was given 1 000 mg per fractio for A group, 3 times a week(Monday, Thursday, Friday)in solution of 100 mL normal saline, iv drip within 30 minutes; placebo was given for B group. Three-dimensional conformal radiotherapy was started 30--60 minutes after completion of the infusion, 5 times each week, and radiotherapy was given by conventional schedule: 2.0 Gy per fraction, 5 times per week to a total dose of 60--70 Gy/6--7 weeks. RESULTS: The rate of CR and total effective rate(CR+PR) were significantly higher in Gruop A than those in Group B. The median doses by which the effects had reached PR and CR respeetively were lower in Group A than in Group B. There was no significant difierence between the two groups in the rate of adverse reaction, P= 0.54. CONCLUSIONS: CMNa is radiosentizative to NSCLC in stage Ⅲ with 3-DCRT. It could increase the therapeutic result of NSCLC without severe toxicities.
