摘要
目的:探索在现有体制下开展临床药学服务的内容和方法。方法:专职临床药师在老年内分泌科广泛参与临床药学实践活动,通过参与临床医生查房、总结典型药历、不良反应监测、提供药物信息、参与用药方案的制定等多种形式开展全程化药学服务。结果:临床药师能够为医护人员提供药品信息,对安全用药提出合理化建议,为患者进行用药指导,能够成为医生和患者的得力助手。结论:实行临床药师制度是大势所趋,临床药学工作是医院药学服务新的发展方向,广大药师仍需切实加强自身综合素质的培养,最大限度地为广大患者服务。
Objective：To explore the content and method of clinical pharmacy service under the current hospital system.Methods：Clinical pharmacist participated in clinical pharmacy practice and explored whole range of pharmaceutical care in senile endocrine department by attending clinician ward rounds,summarizing typical medication cases,analyzing adverse drug reactions,providing drug information,helping to draw up new therapy projects.Results：Clinical pharmacist can provide drug information and rational proposals for doctors and nurses,and can carry out correctly medicinal direction,as doctor＇s assistant and patient＇s helper.Conclusion：Clinical pharmacy service is a new direction of pharmaceutical care.However,most of the clinical pharmacists should work harder to strengthen their abilities.
