单相波、双相方波除颤106例对比分析 被引量:1

Effects of single-phase wave and biphasic-square wave defibrillators: a comparative study of 106 cases

摘要 目的探讨双相方波除颤的临床应用价值。方法将106例心室颤动(VF)病例分为单相波组和双相方波组,对比分析两组的除颤有效率、自主循环恢复率、除颤后的心肌酶学改变、心电图改变及皮肤灼伤情况。结果双相方波组除颤有效率、自主循环恢复率为96.4%和58.2%,均高于单相波组(分别为70.6%和35.3%,均P<0.05)。双相方波组的皮肤灼伤率为16.4%,显著低于单相波组(66.7%,P<0.01)。双相方波组血清肌酸激酶(CK)、肌酸激酶同工酶(CK-MB)和乳酸脱氢酶(LDH)分别为(378.2±106.5),(59.78±23.7),(347.9±90.9)U/L,均显著低于单相波组([1154.7±170.75),(172.2±58.3)和(777.7±289.2)U/L,均P<0.01]。双相方波组的ST段压低率为28%(9/32),显著低于单相波组[61%(11/18),P<0.01]。结论双相方波除颤的效果优于单相波。 Objective To investigate the clinical values of single-phase wave and biphasic-square wave defibrillators in treatment of ventricular fibrillation (VF). Method 106 VF patients were randomly divided into 2 sex, age, etiology, and duration of VF-matched groups: single-phase wave group (n=51) and biphasic-square wave group (n=55). The defibrillation success rate, recovery rate of spontaneous circulation, skin burn rate, serum levels of myocardial enzyme, and ST segment depression rate were compared. Resu Its The defibrillator success rate and recovery rate of spontaneous circulation of the biphasic-square wave group were 96.4% and 58.2% respectively, both significantly higher than those of the single-phase wave group (70.6% and 35.3% respectively, both P〈0.05). The serum levels of creatine kinase (CK), CK-MB, and lactic dehydrogenase of the biphasic-square wave group were (378.2 ±106.5), (59.78 ± 23.7), and (347.9 ± 90.9) U/L respectively, all significantly lower than those of the single-phase wave group [(1154.7± 170.75), (172.2± 58.3), and (777.7± 289.2) U/L, all P〈 0.01). The ST segment depression rate of the biphasic-square wave group was 28% (9/32), significantly lower than that of the single-phase wave group (61%, 11/18, P〈 0.01). Gonc lusion Biphasic-square wave defibrillation is more effective than single-phase wave defibrillation in treatment of VF.
