摘要
新型农村合作医疗是非营利的公益组织,与公司相比存在着治理缺陷。在借鉴国内外社区医疗保险计划的绩效评价方法的基础上,文章构建一个新农合评价指标体系,旨在引进新农合的激励和约束机制;然后运用该评价体系对西部15个第一、二批试点县的新农合的运行过程和效果进行了综合分析和评价。研究表明,西部地区的新农合政府补贴到位率较低,平均为87%;医疗费报销比率过低,乡卫生院平均报销率为46%;起付线也不合理。最后,提出提高新农合运行绩效的政策建议和措施。
Compared with company, as a not-for-profit commonweal organization the new rural cooperative medical scheme（RCMS） has the defect of governance. Referring to the method of performance evaluation of community-based health insurance schemes, the paper establishes the index system aiming to introducing an incentive and restraining mechanism of new RCMS. The paper also analyzes and evaluates the operation process and effectiveness of 15 pilot counties in four provinces. The results demonstrate that the subsidies of government reached the foundation later than scheduled, on average only 87% of subsidies reach the foundation account as Scheduled; the rates of reimbursement of medical costs are also too low, which in town hospital is 46 % ;the deductibles are not reasonable. Finally, the paper puts forward the suggestion and measures that can improve the operation performance of new RCMS.
