摘要
目的比较两种不同剂量肾上腺素在小鼠心肺复苏中的疗效,寻找小鼠心肺复苏中较合理的肾上腺素剂量。方法30只小鼠经食道快速起搏心室诱发室颤、建立心脏停搏模型。起搏开始后4min将小鼠随机分成三组(n=10):对照组(C组)、低剂量肾上腺素组(L组)和高剂量肾上腺素组(H组),分别经动脉注射药物(生理盐水、肾上腺素0.04mg/kg或肾上腺素0.2mg/kg)1次,同时开始胸外心脏按压及机械通气,观察自主循环恢复情况,10min无效则放弃复苏。自主循环恢复的小鼠连续监测心电和血压60min,观察血流动力学、呼吸恢复情况及生存时间。结果L、H组小鼠自主循环恢复率均显著高于C组(分别为10/10,10/10,和3/10,P<0.01),L、H组间比较差异无统计学意义。L组小鼠的生存时间明显长于H组和C组(P<0.05)。结论低、高剂量肾上腺素均可提高小鼠自主循环恢复率,但低剂量肾上腺素的疗效优于高剂量肾上腺素,可延长自主循环恢复小鼠的生存时间。
Objective The purpose of this study was to compare the effects of two doses of epinephrine and identified an optimal dose of epinephrine during cardiopulmonary resuscitation (CPR) in a mouse model. Method Transoesophageal ventricular pacing was performed so as to elicit ventricular fibrillation in 30 mice. At Four minutes from the initiation of ventrieular pacing, the animals were prospectively randomized into three groups ( n = 10 ) and receive saline ( Sal - gro, 0.2 mL i. a. ) , low dose of epinephrine ( Lepi - gro, 0.04 mg/kg i. a. ) , high dose of epinephrine ( Hepi - gro, 0.2 mg/kg i. a. ) , then CPR was initiated. Restoration of spontaneous circulation ( ROSC ) was observed. When there was a failure to restore spontaneous circulation after 10 min, resuscitation efforts were discontinued. Electrocardiogram and blood pressure of resuscitated animals were invasively monitored for an additional 60 minutes. Hemodynamies and the restoration of spontaneous breathing and survival time were observed and immedi- ately recorded. Result Rates of ROSC in Lepi - gro and Hepi - gro were significantly higher than in Sal - gro ( 10/10, 10/10 vs 3/10, P 〈 0.01 ) , but there was no significant difference between Lepi - gro and Hepi - gro. Survival time in Lepi - gro was longer than that in Hepi - gro or in Sal - gro ( P 〈 0. 05 ). Conclusion Though both low and high dose of epinephrine can enhance the rates of ROSC, low dose of epinephrine results in a longer survival time and a better outcome of CPR compared with high dose of epinephrine in this mouse model.
