摘要
目的:探讨儿童紫癜性肾炎(HSPN)的临床和肾组织病理改变特点及其关系。方法:回顾性分析1995年12月～2007年12月进行过肾活检的105例HSPN患儿的临床和肾组织病理改变情况。结果:105例HSPN患儿中,单纯血尿组18例(17.14%),血尿及蛋白尿组61例(58.10%),肾病综合征组26例(24.76%);三组间的Scr、BUN、IgA、IgE、CRP和C3水平均无统计学差异(P均>0.05),血浆白蛋白在各组间存在统计学差异(P均<0.01)。病理上ISKDC分级结果为:Ⅱa级8例(7.62%),Ⅱb级21例(20.00%),Ⅲa级33例(31.43%),Ⅲb级38例(36.19%),Ⅳ级5例(4.76%);三组间比较有统计学差异(χ2=44.51,P<0.01)。肾小管间质病变的组织学分级结果为:(-)级34例(32.38%),(+)级64例(60.95%),(++)级6例(5.71%),(+++)级1例(0.95%),(++++)级0例(0%);三组间比较有统计学差异(χ2=40.59,P<0.01)。结论:儿童HSPN临床表现多样,肾病理改变相对较轻,且两者之间存在密切关系。
Objective： To investigate the clinical and renal pathologic features of Henoch- Schonlein Purpura Nephritis （HSPN） in Children and the relationship between them. Methods：A retrospective review of 105 cases of HSPN who had been performed renal biopsy within 12 years（1995 to 2007） were conducted to analyse their clinical manifestations and renal pathologic features. Results： In 105 children with HSPN, there were 18 （ 17.14 % ） children with isolated hematuria 161 （58.10 % ） with hematuria and proteinuria, and 26 （24.76 % ） with nephrotic syndrome. Among the above three groups, there were no significantly differences of the levels of serum ereatinine, serum urea nitrogen, IgA, IgE, CRP and C3 between every two groups（P 〉 0.05） ; but the plasma - albumin in group B were significantly higher than that in group A（ P 〈 0.01 ）,and significantly lower than that in group C（ P 〈 0.01 ）. Among the pathologic changes of their nephridial tissue, according to ISKDC grading, there were 8 cases（7.62 % ） with Ⅱa, 21 cases （20.00%） with Ⅱ b,33 cases （31.43%） with Ⅲa,38 cases （36.19%） with Ⅲb,and 5 cases （4.76%） with Ⅳa. There was significant difference among the above three groups（ x^2 = 44.51, P 〈 0.01）. According to nephric tubule and interstitium change grading,there were 34 cases（32.38%） with - ,64 cases （60.95%） with ＋ ,6 cases （5.71%） with ＋ ＋, lcases （0.95%） with ＋ ＋ ＋ ,and 0 cases （0%） with ＋ ＋ ＋ ＋. There was also significant difference among the above three groups（ x^2 - 40.59, P〈 0.01）. Conclusion：The clinical manifestation of HSPN in children is multiplex, but its pathologic change is relatively light;and there is a close relation between the clinical manifestation and the renal pathologic change.
